慢性粒细胞白血病患者中西医结合治疗前后舌微循环及骨髓超微结构变化

摘　　要：目的:观察45例慢性粒细胞白血病(CML)患者中西医结合治疗前后的舌尖微循环及骨髓超微结构变化。方法:对45例复方黄黛片联合羟基脲治疗CML患者治疗前后的舌尖微循环及骨髓组织病理、超微结构进行对比观察。结果:CML患者治疗前舌尖微循环超微结构变化包括微血管扩张、内皮肿胀、微血管数量增多;骨髓组织病理表现为中晚幼粒细胞增生明显,且伴有不同程度的骨髓纤维化,超微结构可见中晚幼细胞密集,部分细胞出现破裂。经复方黄黛片联合羟基脲治疗2个月后舌尖组织微循环明显改善,骨髓中白血病细胞明显减少,增生程度减低;但超微结构仍有核固缩等细胞凋亡的形态学改变。结论:本观察结果为CML的中医辨证论治、指导治疗及判断疗效提供了理论依据。Objective:To investigate the tongue tip organization and bone marrow ultrastructural changes for the 45 chronic myeloid leukemia(CML) patients after treatment of integrative medicine.Method:45 CML patients were observed of their tongue tip organizations,microvascular structure,bone marrow pathology and ultrastructure using multi-site microscopic LAU type WX-6 before and after treatment of the compound huangdai tablets and hydroxycarbamide.Results:Tongue microcirculation stuctures change in 45 CML patients with microvascular expansion,endothelial swelling,microvascular cavity volume increased; in bone marrow pathology,promyelocytic cells proliferation significantly,and with varying degrees of bone marrow fibrosis; in the bone marrow ultrastructure,the promyelocytic cells in the late-intensive,most of the structural integrity of cells,some cells rupture. After 2 mongths treatment of compound Huangdai Tablets,the pathological damage of tongue organizations and bone marrow have different degrees of improvement,with reducing the degree of proiferation and leukemic cells and the ultrastructure of nuclear condensation occurs such as morphological changes of apoptosis.Conclusion:By observing the tongue microcirculation andl bone marrow ultrrastructure,we can provide an objective basis to progress TCM syndrome differentiation and guide the treatment of CML patients and determine efficacy.
