摘　　要：目的　探讨小细胞肺癌脑转移综合治疗的疗效，方法　自１９８５．１～１９９３．１，共９８例小细胞肺癌颅内转移患者，按治疗方法不同，分为单纯化疗组（Ａ组），单纯放疗组（Ｂ组），综合治疗组（Ｃ组），Ａ组３０例，Ｂ组３２例，Ｃ组３６ 例，结果Ａ、Ｂ、Ｃ三组的１、２年生存率及中位生存期分别为：Ａ组０，０，３．２个月；Ｂ组２５％ 、９．４％ 、６．５个月；Ｃ组３０．５％ 、１９．４％ 、９．６个月Ｂ、Ｃ两组１、２年生存率及中位生存期明显高于Ａ组，Ａ、Ｂ、Ｃ三组单发转移灶中位生存期分别为为３．４，７．２，１０．８个月。多发转移灶中位生存期分别为２．６、５．８、８．４个月。肺内原发灶控制患者中位生存期分别为４．４，７，１２个月，肺内原发灶未控制患者，中位生存期分别为２．６，５．８，９．２个月。如何提高肺部原发灶控制率，提高小细胞肺癌颅内转移疗效，仍需进一步探讨，结论　在小细胞肺癌的综合治疗中，放疗＋ 化疗可提高患者生存率，只要病人条件允许，全脑预防照射是有积极意义的，可否提高生存率，应作随机分组临床研究。? Objective To report the curative effect for small cell lung cancer with brain metastasis.Methods patients suffering from brain metastasis of small cell lung cancer were reported from January,1985 to January,1993.The patients were divided into three groups:Group A(30 cases)received both chemotherapy only,Group B(32 cases)received radiothrerapy only and Group C(36 cases)received chemotherapy and radiotherapy.Results The 1 and 2 year survival rates and median survival period were 0%,0%,and 3.2 months in group A, 25%,9.4% and 6.5 months in group B and 30.5%,19.4 and 9.6 months in group C respectively.The 1 and 2 year survival rates and median survival period of group B and C were obviously different from that of Group A.For Group A,B and C,the median survival period of single focus were 3.4,7.2,10.8 months respectively,2.6,5.8,8.4 months respectively for mutiple foci, 4.4,7,12 months respectively for patients with controlled primary lesion, and 2.6,5.8,9.2 months respectively for patients with uncontrolled primary lesions.Conclusion Radiotherapy combined with chemotherapy might improve the curative effect for small cell lung cancer with brain metastases.〔
