氯硝西泮静脉注射治疗冲动兴奋患者对照研究

摘　　要：目的:观察静脉注射氯硝西泮对兴奋状态精神疾病患者的疗效和安全性。方法:将患者随机分为研究组(n=110)和对照组(n=108),研究组静脉注射氯硝西泮2～4mg,对照组肌内注射氟哌啶醇10～20mg。观察给药后6h患者安静下来的例数及所需时间、需要防护例数及防护时间、合作例数和修订版外显攻击行为(MOAS)评分。结果:6h后研究组安静下来78例,对照组58例(P<0.01);研究组安静下来需(8.99±5.62)min,对照组(26.78±16.38)min(P<0.01)。研究组需防护26例,对照组44例(P<0.01);研究组防护时间(56.57±56.97)min,对照组(87.52±59.67)min(P<0.01)。研究组合作83例,对照组60例(P<0.01)。研究组MOAS评分较对照组显著降低(P<0.01)。氯硝西泮不良反应多为轻度或中度,无须特殊处理。结论:静脉注射氯硝西泮能快速控制精神疾病患者的兴奋状态,提高护理合作率,降低防护率,缩短防护时间,家属容易接受,安全性大,有推广价值。Objective： To explore efficacy and safety of clonazepam intravenous injection in treatment of acute excitement of psychotic patients. Method：The psychotic patients with acute excitement were randomly divided into the study group （n=110） and the control group （n=108）.The study group received 2~4 mg/d clonazepam intravenous injection treatment while the control groups received 10~21 mg/d haloperidol intravenous injection treatment.The curative effects were assessed after 6 hours. Results：After 6 hours,78 in the study group and 58 in the control group was quiet （P0.01） and the average time when patients took to calm down were （8.99±5.62） and （26.78±16.38） minutes respectively （P0.01）;26 in the study group and 44 in the control group were given restraint and the average restraint time was （56.57±56.97） and （87.52±59.67） minutes respectively （P0.01）;83 in the study group and 60 in the control group cooperated with the treatment;The study group showed significant lower （modified overt aggression scale） score than the control groups （P0.01）;There were no significant differences on side effects between the two groups （P0.05）. Conclusion：Clonazepam intravenous injection is effective and safe in treatment of the acute excitement of psychotic patients.
