p53和survivin在子宫内膜样腺癌中的表达及相关性分析

摘　　要：目的探讨抑癌基因p53和凋亡抑制因子survivin在子宫内膜样腺癌中的表达与临床病理特征的关系及相关性。方法应用免疫组化SP法检测p53和survivin在石蜡包埋的子宫内膜样腺癌及正常子宫内膜组织各40例中的表达。结果 p53和survivin在子宫内膜样腺癌中的阳性表达率均高于正常子宫内膜(均P<0.05);p53及survivin阳性表达的腺癌的淋巴结转移率明显高于阴性表达者(P<0.05)。survivin阳性率在子宫内膜样腺癌的高分化和低分化组之间的差异有统计学意义(P<0.05)。p53、survivin在子宫内膜样腺癌的表达呈正相关(r=0.196,P<0.05)。结论突变型p53、survivin在子宫内膜样腺癌中的表达高于正常子宫内膜;survivin在子宫内膜样腺癌中的表达与肿瘤分化程度及淋巴结转移有关;突变型p53和survivin在子宫内膜样腺癌中的表达有协同作用;p53突变与survivin过度表达可能是子宫内膜样腺癌发生、发展的机制之一。Objective To investigate the expression and correlation of p53,survivin in endometrioid adenocarcinoma and their relationship with clinicopathologic features.Methods The expression of p53 and survivin in 40 cases of endometrioid adenocarcinoma and 40 cases normal endometrium was detected by immunohistochemical SP method.Results The positive rate of both p53（P0.05） and survivin（P0.05） was higher in endometrioid adenocarcinoma than that in normal endometrium.The rate of lymph node metastasis in endometrioid adenocarcinoma with p53 or Survivin positivity was higher than those with corresponding negativity（P0.05）.The positive rate of survivin increased in poorly differentiated endometrioid adenocarcinoma（P0.05）.There was a positive correlation between the expression of p53 and survivin in endometrioid adenocarcinoma（r=0.196,P0.05）.Conclusions The mutated p53 and survivin are expressed higher in endometrioid adenocarcinoma than that in normal endometrium.The expression of survivin in endometrioid adenocarcinoma is related to tumor differentiation and lymph node metastasis.The expression of mutated p53 and survivin has synergistic effect in endometrioid adenocarcinoma.The mutation of p53 and survivin over-expression may be a mechanism in occurrence and progression of endometrioid adenocarcinoma.
