奥沙利铂联合5-氟尿嘧啶和醛氢叶酸钙时辰方案治疗晚期胃癌63例临床观察

摘　　要：目的观察奥沙利铂（L—OHP）联合5-氟尿嘧啶（5-Fu）、醛氢叶酸钙（CF）方案时辰给药治疗晚期胃癌的疗效和不良反应。方法采用L-OHP／CF／5-FU时辰方案治疗63例晚期初治胃癌患者，L-OHP35mg／（m^2·d），5-Fu500mg／（m^2·d），CF200mg／d。L—OHP给药时间为10：00-22：00，持续输注12h，给药高峰16：00；5-Fu、CF每天给药时间为22：00一次日10：00，持续输注12h，给药高峰凌晨4：00，连续给药4d。每21d为1个周期，至少用2个周期。结果全组患者共完成346个周期化疗，可评价疗效53例。其中完成6个周期47例，4个周期16例。CR2例（3．77％），PR27例（50．94％），SD10例（18．88％），PD14例（26．42％），总有效率（CR＋PR）54．72％。TTP为6．5月（2～52月），中位生存时间为9．8月（4～60月）。最常见的不良反应为血液学毒性、外周神经毒性、胃肠道毒性，但均以I度为主，未出现严重不良反应。结论L—OHP／CF／5-FU时辰方案是晚期胃癌的有效方案，与常规给药方法相比，有可能进一步提高疗效，降低化疗的毒副反应，提高患者的生活质量，值得临床推广使用。Objective To investigate the efficacy and safety of combination of oxaliplatin,5-fluorouracil and folonic acid（L-OHP/CF/5-FU） in form of chronotherapy for advanced gastric cancer. Methods 63 cases patients with advanced gastric cancer received L-OHP/CF/5-FU regimen by multichannel, programmable pumps. Oxaliplatin was given in 35 mg/（m2 · d）, from 10：00 to 22：00, peak at 16：00,dl-4; Both 5-FU 500 mg/（m2 · d） and CF 200mg/d were given from 22：00 to 10：00, peak at 4.00 dl-4; ,Every 21 days was one cycle, each case received at least 2 cycles. Results 2 complete and 27 partial remissions were observed in all 53 patients with overall response rate of 54.72%. Stable disease was observed in 10 patients（18.88%） and progressive disease in 14 patients（26.42%）. The median Time-To-Progress was 6.5 months（2-52 months） and the median overall survival time was 9.8 months（4-60 months）. The main toxicity included myelosuppression, neurotoxicity and nausea/vomiting. Most of them were grade I . Conclusion Chronotberapy with oxaliplatin, 5-fluorouracil and folonic acid is effective and safe in advanced gastric cancer. It may be applied to extensive patients for better quality of life.
