Treatment of Atlas Fracture by Operation under ISO-C-3D Navigation and Its Clinical Significance

摘  要:探讨导航技术在手术治疗寰椎骨折中的应用问题及其临床意义。使用C臂透视导航设备对22例不稳定性寰椎骨折的患者行导航下螺钉内固定术。9例骨折采用寰椎侧块螺钉固定,13例骨折采用枕颈融合。导航下共植入椎弓根螺钉70枚,侧快螺钉18枚。68枚(97.7%)椎弓根螺钉为1类螺钉,2枚(2.3%)椎弓根螺钉为2类螺钉,没有3类螺钉;18枚侧快螺钉置钉准确,无侵犯横突孔、神经孔或小关节的现象。导航辅助置入螺钉治疗不稳定性寰椎骨折具有较高的准确性和安全性。To evaluate the preliminary clinical outcome of the ISO--C--3D navigation system used in the treatment of atlas fracture by operation and discuss its clinical significance. 22 patients with atlas fractures were treated with screw fixation under the guidance of an ISO--C--3D navigation system. 9 patients underwent C1 lateral mass and C2 pedicle screw fixa- tion and 13 patients underwent occipitocervical fusion. 70 pedical screws and 18 lateral mass screws were inserted under ISO--C--3D navigation. Of the 70 pedical screws inserted , 68 pedical screws (97. 7%) were classified as Grade 1, 2 pedical screws (2.3 %) were classified as Grade 2, no pedical screws (0 %)were classified as Grade 3. None of the 18 lateral mass screws showed evidence of foramen transversariurn, neural foramen, or facet joint violation. Assistance with ISO --C--3D navigation system, it makes more precise, safe, time--saving and less X--ray exposure to do cervical pedical screw fixation on the atlas fractures.
