影响自贡地区防盲白内障手术效果因素分析

摘　　要：目的：探讨自贡地区贫困白内障患者接受复明手术后视力恢复效果，分析影响因素。方法：回顾性分析配合四川省防盲办白内障复明工程，于表面麻醉联合球周麻醉下进行无缝线白内障囊外摘除联合人工晶体植入术300例（300眼），统计分析手术并发症，术后3个月视力情况。结果：①并发症：后囊膜破裂20眼（6．7％），上方虹膜反复脱出4眼（0．1％），切口渗漏5眼（1．6％），角膜水肿64眼（21．3％），葡萄膜炎23眼（7．6％），前房出血8眼（2．6％），继发性高眼压10眼（1．4％）；②术后视力：术后随、方3个月，矫正视力≥0．5者248眼（82．6％），0．3～0．4者42眼（14．O％），〈0．3者10眼（3．3％）。结论：白内障手术情况复杂，眼部原发病、手术并发症及手术操作技巧等均是影响复明手术效果的因素之一。正确应对各种影响因素，以提高贫困白内障手术的复明率。Objective To analyze the causes influencing the effect of cataract surgery preventing from blindness in Zigong. Method Analysing the complications and the recovery of the 300 patients＇ eyesight, who had undergone the sutureless small inci- sion extra capsular cataract extraction and intraocular lens implantation at the anesthesia of surface and peribulbar in our hospital in July to December in 2009 with the Sichuan Provincial Office of cataract blindness prevention projects. Results （1）Complications：posterior capsule rupture in 20 eyes （ 6. 7% ） , repeated iris prolapse of the top in 4 eyes （0. 1% ）, wound leak in 5 eyes （ 1.6% ） , corneal edema in 64 eyes （21.3%） ,grape inflammation in 23 eyes （7.6%） ,anterior chamber hemorrhage in 8 eyes （2. 6% ） ,secondary ocular hypertension in 10 eyes （ 1.4% ）. （2）Visual acuity were followed up in March, corrected visual acuity ≥ 0. 5 in 248 eyes （ 82. 6% ） , 0. 3 - 0. 4 in 42 eyes （14. 0% ） , 〈0. 3 in 10 eyes （3.3%）. Conclusion Cataract surgeries are complicated the original diseases of the patients＇ eyes,surgical complications and operative techniques and skills are the factors affecting sight restoring. The correct response to various factors, to increase the rate of restoring the patients＇ sight in the poor areas ,do deal with the factors properly and calmly.
