进展期胃癌根治术后腹膜复发的危险因素分析

摘　　要：目的:探讨进展期胃癌根治术后腹膜复发的相关危险因素。方法:回顾性分析我院根治术后进展期胃癌患者471例的资料,进行术后系统随访,以Logistic回归分析5年内腹膜复发的因素。结果:471例患者五年随访率88.9%,随访资料完整的419例患者中,共有153例发生腹膜复发(36.5%)。术后2年内115例复发(75.2%),3年内142例复发(92.8%)。Logistic回归分析显示:肿瘤浸润深度(P=0.017,OR=3.077)和术后早期腹腔内化疗(P=8.012,OR=3.056)是影响胃癌术后五年腹膜复发的独立危险因素。结论:肿瘤浸润深度和术后早期腹腔内化疗是影响胃癌术后五年腹膜复发的独立危险因素。Objective：To evaluate the risk factors of peritoneal recurrence after curative resection of advanced gastric cancer.Methods：A total of 471 patients with advanced gastric cancer in our hospital who have curative resection were enrolled in this study by a retrospective analysis.After five years postoperative follow-up,Logistic regression were used for analysis the factors of peritoneal recurrence.Results：471 patients were followed up for five years,419 patients with complete data（88.9%）.There were 153 patients had peritoneal recurrence（36.5%）.At 2 years,115 patients had recurrence（75.2%）;at 3 years,142 patients had recurrence（92.8%）.Logistic regression analysis showed that：depth of tumor invasion（P = 0.017,OR= 3.077）,early postoperative intraperitoneal chemotherapy（P=8.012,OR=3.056） were independent risk factors of peritoneal recurrence for gastric cancer after radical gastrectomy.Conclusion：The depth of invasion,early postoperative intraperitoneal chemotherapy were independent risk factors of peritoneal recurrence for gastric cancer after radical gastrectomy.
