血清肿瘤标志物联合检测在胰腺癌诊断中应用

摘　　要：目的:探讨血清肿瘤标志物癌胚抗原(carcinoembryonic antigen,CEA)、癌抗原19-9(carbohydrate antigen,CA19-9)和细胞角蛋白19片段(cytokeratin-19 fragments,CK19)联合检测在诊断胰腺癌中的价值。方法:检测50例确诊胰腺癌患者(胰腺癌组)及150例健康体检者(对照组)外周血清中CEA、CA19-9与CK193种肿瘤标志物水平,并对结果进行分析。结果:胰腺癌组血清中CEA、CA19-9与CK19含量高于对照组,差异有统计学意义(P<0.01)。单项检测时CEA、CA19-9与CK19的敏感度分别为56.0%、72.0%和74.0%,特异性分别为75%、78%和88%;联合检测时敏感度为93.0%,特异性为100%。结论:CEA、CA19-9与角蛋白CK19联合检测敏感度和特异性均高于单项检测,联合检测较单项血清标志物检测能提高胰腺癌的诊断率。Objective To explore the clinical diagnostic value of combined detection of carcinoembryonic antigen（CEA）,carbohydrate antigen（CA19-9） and cytokeratin-19 fragments（CK19） to pancreatic carcinoma.Methods The levels of CEA,CA19-9 and CK19 were detected and analyzed in 50 patients with pancreatic carcinoma and 150 healthy volunteers.Results The levels of tumor markers in pancreatic carcinoma group were significantly higher than those in control group （P0.01）.In pancreatic carcinoma group,the sensitivities of CEA,CA19-9 and CK19 were 56.0%,72.0% and 74.0%,respectively,and the specificities of CEA,CA19-9 and CK19 were 75%,78% and 88%,respectively The sensitivity and specificity of combined detection were 93% and 100%.Conclusion The sensitivity and specificity of combined detection of CEA,CA19-9 and CK19 are higher than those of the single detection,and the combined detection can improve the detection rate of pancreatic carcinoma.
