摘 要:目的观察近视患者白内障术后的眼压动态变化,评价眼轴长度和术前基础眼压对它的影响。方法随机选取术前眼压正常病例75例(75眼),分为正常眼轴组(对照组)25例、低度轴性近视组(24mm<眼轴长度≤27mm)30例、高度轴性近视组(眼轴长度>27mm)20例。进行超声乳化白内障摘出人工晶状体植入术,使用Goldmann压平眼压计测量眼压,分别于术前和术后2-4h、6-8h、12-14h、24h、48h测量眼压。结果同一时间点各组之间眼压比较显示:高度轴性近视组术后6-8h眼压及12-14h分别为(33.94±13.69)mmHg(1kPa=7.5mmHg)、(28.03±9.61)mmHg,与对照组(23.27±7.23)mmHg、(27.72±5.27)mmHg及低度轴性近视组(26.77±9.81)mmHg、(22.57±6.23)mmHg比较,差异均有统计学意义(均为P<0.05)。低度轴性近视组术后6-8h眼压、12-14h眼压、与术前眼压(15.83±2.76)mmHg均具有相关性(r=0.450,P=0.013);12-14h眼压与术前眼压具有相关性(r=0.413,P=0.023)。高度轴性近视组术后6-8h眼压与术前眼压(16.80±2.61)mmHg具有相关性(r=0.642,P=0.002;r=0.611,P=0.002);术后24h眼压(21.35±7.58)mmHg与术前眼压亦具有相关性(r=0.702,P=0.001)。结论高度轴性近视(眼轴长度>27mm)患者白内障术后早期存在眼压一过性升高;近视患者术后早期眼压变化与术前基础眼压呈直线相关,术前眼压越高,术后早期眼压越容易升高。Objective To investigate length of ocular axis and preoperative basic intraocular pressure(IOP)on dynamic changes of IOP by observing its changes after phacoemulsification in myopic patients.Methods About 75 cases(75 eyes)with normal preoperative IOP were enrolled in this prospective study and randomized into 3 groups,control group(25 cases),low ocular axis myopic group(30 cases;24 mmlength of ocular axis≤27 mm)and high ocular axis myopic group(20 cases;Length of ocular axis27 mm).All these patients underwent phacoemulsification with posterior chamber lens implantation.The IOP was measured preoperatively and at postoperative 2 to 4 hours,6 to 8 hours,12 to 14 hours,24 hours and 48 hours.Results Comparison of IOP among different groups at the same time point showed that IOP at postoperative 6 to 8 hours and 12 to 14 hours were(33.94±13.69)mmHg(1 kPa=7.5 mmHg)and(28.03±9.61)mmHg in high ocular axis myopic group,(23.27±7.23)mmHg and(27.72±5.27)mmHg in control group,(26.77±9.81)mmHg and(22.57±6.23)mmHg in low ocular axis myopic group.There were significant differences for high ocular axis myopic group compared with control group or low ocular axis myopic group(all P0.05).In low ocular axis myopic group,there were correlation with IOP at postoperative 6 to 8 hours with preoperative IOP(15.83±2.76)mmHg(r=0.450,P=0.013),and IOP at postoperative 12 to 14 hours with preoperative IOP(r=0.642,P=0.002).In high ocular axis myopic group,there were correlation with IOP at postoperative 6 to 8 hours with preoperative IOP(16.80±2.61)mmHg(r=0.642,P=0.002),and IOP at postoperative 12 to 14 hours with preoperative IOP(r=0.611,P=0.001).Conclusions There is transitional increase of IOP in patients with high ocular axis myopia at the early stage after phacoemulsification.There is linear correlation of postoperative IOP changes and preoperative basic IOP in myopic patients.Postoperative IOP at early stage is easier to increase with higher preoperative
