FOLFIRI方案一线治疗晚期胃癌临床观察

摘　　要：目的观察我科2009年1月．2010年7月收治30例晚期胃癌患者以伊立替康联合氟尿嘧啶、亚叶酸钙双周（FOLFIRI）方案治疗，观察其近期疗效及安全性。方法CPT-11：180mg／m^2静滴60．90mind1；CF：200mg／m^2静滴2hd1，d2；CF静滴结束后立即5FU400mg／m^2静脉推注d1；5-FU：600mg／m^2持续微泵静脉推注22h，d1，d2。每2周重复，28天为1个周期，每2个周期评价疗效，不足2周期的观察毒性，获CR及PR患者4周后确认。结果全组29例可评价疗效，CR4例（13．79％），PR10例（34．48％），SD11例（37．93％），PD4例（13．79％）。有效率RR（CR＋PR）14（48．27％）；不良反应较轻，以I，II度多见。结论以CPT-11为主的（FOLFIRI方案）一线化疗晚期胃癌具有较高的疾病控制率，且安全性好，不良反应可控制，值得临床进一步验证。Objective To observe the therapeutic effect and safety of FOLFIRI regimen in the treatment of 30 advanced gastric carcinoma patients during January 2009 and July 2010. Methods CPT-11 was intravenous dripped for 60-90 minutes at the dose of 180 mg/m^2; CF was intravenous dripped for 2 hours at the dose of 200 mg/m^2 on day l and day 2; 5FU was intravenous injected at the dose of 400 mg/m^2 on day 1 immediately after the CF administration; then 5FU was intravenous dripped for 22 hours at the dose of 600 mg/m^2 on day 1 and day 2. The regimen was repeated every 2 weeks, and 28 days were considered as 1 cycle. The therapeutic effects were evaluated every 2 cycles. Results In the 29 cases, 4 cases were CR （13.79 % ）, 10 cases were PR （34.48 % ）, 11 cases were SD （37.93 % ）, 4 cases were PD （13.79 % ）. The RR （CR＋PR） was 48.27 % （14 cases） The side effects were relatively light. Conclusions The FOLFIRI regimen is effective and safe in the treatment of advanced gastric carcinoma, and deserve the further validation in the clinical practice.
