中国韦氏儿童智力量表修订本简式在注意缺陷多动障碍儿童的应用研究

摘　　要：目的探讨中国韦氏儿童智力量表修订本(Chinese-Wechsler intelligence scale of children,C-WISC)简式在注意缺陷多动障碍(attention deficit hyperactivity disorder,ADHD)儿童患者中的应用价值,并对临床常用的C-WISC简式四合一、五合一及六合一组合进行效度检验。方法对符合美国精神障碍诊断与统计手册第4版(diagnostic and statistical manual of mental disorder 4th edition,DSM-IV)注意缺陷多动障碍标准的门诊患儿47例进行C-WISC全式智商测验;同时选择11项分测验中的7项进行不同的简式测验组合,采用回归法计算9种四合一、9种五合一以及六合一的简式智商,分析全式与简式智商的一致性。结果 ADHD患儿组的5种四合一、5种五合一、六合一简式与全式智商的均值差异均无统计学意义(P>0.05),而另外4种四合一和4种五合一简式组合的智商分别与全式智商的均值差异均有统计学意义(P<0.05)。19种简式智商与全式智商的相关性均较好(r为0.64～0.90,P均小于0.01),其中相关系数大于0.86的简式智商有2种四合一、1种五合一以及六合一;将智商按高智商、正常智商、低智商3个等级划分,各简式智商与全式智商的分等级符合率在68.1%～80.9%之间。结论 C-WISC简式总体来说可大致评估ADHD儿童的智力,但临床应用中应选择与全式测验一致性更好的简式组合。Objective To investigate the utility value of the short forms of Chinese-Wechsler intelligence scale of children（C-WISC） in attention deficit hyperactivity disorder（ADHD） patients and to examine the validity of the four-subtest,the five-subtest and the six-subtest of short forms of C-WISC in Chinese ADHD patients.Methods Forty-seven ADHD patients diagnosed according to the diagnostic and statistical manual of mental disorder 4th edition（DSM-IV） criteria were studied.Each subject was administered a complete C-WISC.The full scale intelligence quotient（IQ） was calculated using all verbal and performance subjects.Seven subtests of 11 subtests were selected as different short forms of C-WISC.Short forms IQ of nine kinds of four-subtest,9 kinds of five-subtest and six-subtest were calculated and converted to IQ scores by using regression formulas.Short forms IQ were then compared with full scale IQ.Results There was significant difference in the mean intelligence scores in 4 kinds of four-subtest and 4 kinds of five-subtest between short form IQ and full scale IQ（P0.05）.However,there were no significant differences between short form IQ and full scale IQ in 5 kinds four-subtest,5 kinds five-subtest and one six-subtest（P0.05）.There were good correlations between 19 kinds of short forms and the complete battery of full scale IQ（r=0.64～0.90,P0.01）.The r value in 2 kinds of four-subtest,1 kind of five-subtest,and six-subtest were greater than 0.86.IQ scores could be further divided into three ranks：high,normal and low ranks.Ranked coincidence rates between full scale IQ and short forms IQ were 68.1%～80.9%.Conclusions Overall results support the utility of short forms in ADHD patients when intelligence is estimated approximately.It would be better to use the short forms which have high consistence with full scale of C-WISC during the clinical practice.
