淋巴结Runx3基因表达对胃癌患者生存的影响

摘　　要：目的 观察转移性和非转移性淋巴结组织runt相关转录因子基因3（Runx3）表达对胃癌患者生存分析的影响.方法 应用半定量逆转录-聚合酶链反应（RT-PCR）法检测101例胃癌根治术患者淋巴结标本的Runx3 mRNA表达水平,术后随访所有患者5年以上.结果 转移性及非转移性淋巴结标本中均可观察到与所设计片段大小一致的Runx3 mRNA扩增产物电泳条带,但前者的比较模糊,其扩增图像的灰度均值转移性淋巴结组（0.29±0.09）显著低于非转移性淋巴结组（0.61±0.18,P＜0.01）;以淋巴结Runx3 mRNA表达的中位数0.362为参考,将101份淋巴结标本分为低表达（≤0.362）和高表达（＞0.362）两组进行Kaplan-Meier方法等统计学分析,发现淋巴结Runx3 mRNA低表达不仅与胃癌的淋巴结转移、远处器官转移、较晚临床病理分期及深度浸润等不良临床病理因素相关,还与胃癌患者较低的5年生存率和总生存率明显相关（P＜0.05）.结论 淋巴结Runx3基因的低表达与胃癌的高侵袭性及其不良预后密切相关.Objective To investigate the influence of Runt-related transcription factor gene 3（ Runx3 ） gene expression in metastatic or no-metastatic lymph nodes （LNs） on survival of patients with gastric cancer.Methods Semiquantitative reverse transcription-polymerase chain reaction （RT-PCR） was used to detect the expression level of Runx3 mRNA in the LNs taken from surgically resected specimens of 101 patients with gastric cancer.All patients were followed up more than five years after radical gastrectomy.Results The PCR bands of Runx3 mRNA,that fragment in size was as the same as the design,were reviewed in LNs with or without metastasis,and the band of former was more indefinite.The arithmetic mean on gray scale bands of Runx3 in the LN group with metastasis was significantly lower than in the LN group without metastasis （0.29 ±0.09 vs 0.61 ±0.18 ,P 〈0.01 ）.According to the median （0.362） of Runx3 mRNA in the LNs,the 101 cases were separated into two groups：the lower expression group （ ≤0.362） and the overexpression group （ 〉 0.362 ）.Kaplan-Meier analysis revealed that the loss of Runx3mRNA expression in the LNs was not only associated with the poor clinicopathological factors such as lymph node metastasis,distant organ metastasis,later clinicopathological stages and deep infiltration,etc.,but also the lower five-years survival rate and cumulative survival rate of patients with gastric cancer.Conclusion There was a significant correlation of reduced expression of Runx3 gene in the LNs with invasion and poor prognosis of gastric cancer.
