摘 要:目的 观察干细胞关键转录因子Oct4在膀胱移行细胞癌中的表达,探讨Oct4的表达与膀胱癌病理分级和临床分期的关系.方法 分别用免疫组织化学和实时定量逆转录-聚合酶链反应(RT-PCR)检测74例膀胱癌组织、32例癌旁组织及24例正常膀胱组织病理标本中的Oct4表达.结果 免疫组织化学显示Oct4在膀胱癌组织的表达明显高于癌旁组织和正常膀胱组织,阳性表达率分别为83.78%、21.87%、0.00%,各组差异有统计学意义(P<0.05),Oct4在膀胱癌不同病理分级中的阳性表达率分别为:G1组66.67%、G2组87.50%、G3组100.00%,在淋巴结转移组和非淋巴结转移组的阳性表达率为92.3%、79.1%,随着病理分级的升高和淋巴结转移的出现,Oct4表达水平逐渐升高,各组间差异有统计学意义(P<0.05),但不同分期的膀胱癌组织中Oct4表达差异无统计学意义(P>0.05);实时定量RT-PCR结果显示Oct4在膀胱癌组织中的表达是癌旁组织的2.00倍,是正常膀胱组织的3.41倍,各组间差异有统计学意义(P<0.05),但不同分期的膀胱癌组织中Oct4表达差异无统计学意义(P>0.05).结论 Oct4的表达与膀胱癌的分级和淋巴结转移密切相关但与临床分期无关,Oct4的过高表达可能在膀胱癌的发生发展中起重要作用.Objective To study the expression and signifiance of Oct4 in bladder transitional cell carcinoma (BTCC),and the relation of its expression with the pathological types and clinical stages of BTCC.Methods Immunohistochemisty and real-time reverse transcription-polymerase chain reaction (RT-PCR) were used to detect the expression of Oct4 in 74 cases of BTCC tissues,32 cases of pericarcinomatous tissues and 24 cases of normal bladder tissues.Results The immunohistochemisty revealed that the expression of Oct4 in BTCC was significantly higher than in pericarcinomatous tissues and normal bladder tissue (P〈0.05 ).The positive rate of Oct4 in BTCC tissues,pericarcinomatous tissues and normal bladder tissues was 83.78%,21.87% and 0.00%,respectively,and that in the pathological grades G1,G2,and G3 was 66.67%,87.50% and 100.00% respectively.The positive rate of Oct4 in the two groups with lymphatic metastasis and negative lymphatic metastasis was 92.3% and 79.1%,respectively.Moreover,with the advancing grades and appearances of lymph node metastasis,the expression of Oct4 was increased gradually,and there was significant difference among groups ( P〈0.05 ).The real-time RT-PCR showed that the expression of Oct4 in BTCC was significantly higher than in pericarcinomatous tissues and normal bladder tissues (P〈0.05 ),but there was no significant difference among clinical stages ( P 〉0.05).Conclusion The positive expression of Oct4 is closely related to the pathology and lymph node metastasis of BTCC but not to clinical stages.The overexpression of Oct4 may play an important role in carcinogenesis and progress of BTCC.
