脑白质疏松症的磁共振血管成像研究

摘　　要：目的应用磁共振血管成像(MRA)观察脑白质疏松症(LA)患者的颅内血管病变情况,进一步探讨LA的血管危险因素。方法选择61例经头颅MRI证实的LA患者和44例无LA的对照组,所有研究对象均行头颅MRA检查并调查其血管危险因素。应用单因素分析和Logistic回归方法分析LA与颅内血管病变和血管危险因素的关系。结果 LA组MRA显示多为颅内混合性循环病变,与对照组比较差异有统计学意义(P<0.01)。LA组与对照组比较在年龄、收缩压、高血压病史等方面均差异具有统计学意义(P<0.05),而性别、吸烟史、饮酒史、脑血管病史和血脂异常等两组比较差异无统计学意义(P>0.05)。二项Logistic回归分析显示:颅内混合性循环病变、高血压病史、年龄为LA的独立危险因素;颅内混合性循环病变与LA的关系最为密切(OR=5.663,P=0.010),高血压病史、年龄与LA也密切相关(P<0.05),收缩压不是LA的独立危险因素(P>0.05)。结论颅内混合性循环病变、高血压病史和年龄是LA的独立危险因素。Objective To study the cerebral artery lesions and vascular risk factors in patients with leukoaraiosis（LA） by magnetic resonance angiography（MRA）.Methods Sixty-one patients（LA group） and 44 old people without LA（control group） were detected by MRI.MRA and vascular risk factors were investigated in all the subjects.The univariate method and logistic regression were used to reveal the association among LA,cerebral artery lesions and vascular risk factors.Results Compared with control group,LA group had more lesions of cerebral mixed circulation（P〈0.01）.There were significant differences in age,systolic blood pressure,and hypertension history between the two groups（P〈0.05）,while no significant differences in sex,smoking history,alcohol history,stroke history and abnormal plasma lipoprotein（P〉0.05）.The logistic regression analysis showed that the lesions of mixed cerebral circulation,hypertension history and elder age were independently associated with LA.The lesions of mixed cerebral circulation had the closest relationship to LA（OR=5.663,P=0.010）.Hypertension history and elder age were also related to LA（P〈0.05）.Systolic blood pressure was not independently associated with LA（P〉0.05）.Conclusion The lesions of cerebral mixed circulation,hypertension history and elder age are independently related to LA.
