摘  要:目的研究乳腺癌雌激素受体(ER)、孕激素受体(PR)表达与细胞核变化的关系,探讨其临床应用价值。方法选择苏州市中医院病理科2002年以来42例乳腺癌标本,总结其ER和PR表达,并作细胞核形态定量图像分析;同时与20例良性病变对比。结果乳腺癌细胞核等圆直径、面积及边缘周长,均较良性对照组为大,差别有统计学意义(P<0.01)。ER+/PR-病例细胞核形态定量与ER+/PR+比较,差别有统计学意义(P<0.05);ER-/PR-病例细胞核形态定量与ER+/PR+比较,差别有统计学意义(P<0.01)。乳腺癌ER和PR可同时或分别失表达,PR失表达病例细胞核形态变量较大,尤其ER和PR同时失表达病例细胞核形态变量更大。结论良、恶性乳腺病变细胞核形态定量差别显著,对鉴别诊断有实用价值。ER和PR的表达结合细胞核形态学测量结果对治疗和预后的评估有意义。Objective To study the expression of hormone receptor and the nuclei morphometry in breast cancer. Methods Several morphometric parameters of cell nuclei were detected by means of a quantitative pathologic technique on 42 cases of breast cancer and 20 cases of benign lesions of the breast respectively. Immunohistochemical SP method was used to detect expression of estrogen receptor(ER) and progesterone receptor(PR) in breast cancer. Results The diameter of nuclei,the area of nuclei and the circumference in cases of breast cancer was found to be significantly larger than those in benign breast lesions(P 〈 0.01 ). Nuclei morphometry in cases of ER +/PR + was significantly different from that in cases of ER +/PR-( P 〈 0.05 ) and ER -/PR - (P 〈 0.01 ) in breast cancer respectively. The morphometric data in ER +/PR - cases of breast cancer were larger than those in ER -/PR + cases of breast cancer, and largest values were observed in ER -/PR - cases of breast cancer. Con- clusion It was of application for morphometry analysis to distinguish benign breast lesions from breast cancer. The expression pattern of estrogen receptor and progesterone receptor in combination with quantitative pathology was of prognostic value in diag- nosis and treatment of breast carcinoma.
