三维适形放疗联合TACE治疗晚期肝癌的临床疗效

摘　　要：目的探讨三维适形放疗联合TACE治疗不能手术切除的原发性肝癌的疗效和不良反应。方法回顾分析2007至2009年收治的不能手术切除的原发性肝癌患者35例,放疗前给予TACE 1~2个疗程,TACE后间隔2周左右给予三维适形放疗,放射剂量为1.8~2Gy/次、1次/d、5次/周,总剂量50.4~60Gy。结果全组35例完全缓解6例、部分缓解22例、疾病稳定6例、疾病进展1例,总有效率为78.5%。1年生存率为79.2%,2年生存率为48.5%,全组患者的中位生存时间为24.8个月。不良反应均可耐受。结论三维适形放疗联合TACE治疗对不能手术切除的原发性肝癌具有较好的疗效,且不良反应可以耐受。Objective To evaluate three-dimensional conformal radiotherapy combined with TACE treatment of unresectable primary liver cancer and adverse reactions.Methods Retrospective analysis of 2007 ~2009 unresectable were treated 35 patients with primary liver cancer,before irradiation TACE 1~2 treatment,TACE interval of 2 weeks after giving three-dimensional conformal radiotherapy,radiation dose is 1.8 to 2 Gy/time,1 time/d,5 times/week,total dose of 50.4 ~ 60 Gy.Results 35 patients with complete remission in 6 cases,22 cases of partial remission,stable disease in 6 cases,1 case of disease progression,the total effective rate was 78.5%.1 year survival rate was 79.2%,2-year survival rate was 48.5% for all patients,the median survival time was 24.8 months.Adverse reactions were tolerable.Conclusion Three-dimensional conformal radiotherapy combined with TACE therapy for unrespectable primary liver cancer with good effect,and adverse reactions can be tolerated.
