摘　　要：目的分析ⅡA分泌型磷脂酶A2(ⅡA sPLA2)在胃癌及癌前病变中的表达,并探讨其与胃癌临床病理特征间的关系。方法 MaxVision免疫组化法检测60例进展期胃癌(AGC)、20例基本正常胃黏膜(NGM)、20例慢性胃炎伴肠化(IM)、20例不典型增生(DYS)病理组织标本中ⅡA sPLA2的表达情况。结果ⅡA sPLA2在IM、DYS和AGC中的阳性率分别为80.0%、55.0%以及41.7%,高于NGM的15.0%(P<0.05)。从IM、DYS到AGC,ⅡA sPLA2的阳性率逐渐降低(P<0.05)。在AGC组,有浆膜层浸润及淋巴结转移的AGC的ⅡA sPLA2阳性表达率明显低于无浆膜层浸润及淋巴结转移的AGC(P<0.05)。结论ⅡA sPLA2表达下调可能是胃癌形成的早期事件,而ⅡA sPLA2表达下调可能与胃癌侵袭、转移有着一定的关系。Objective To investigate the expression of secretory phospholipase A2 group ⅡA（ⅡA sPLA2）in gastric cancer and precarerous lesions,and to identify its correlation with pathologic characteristics of gastric cancer.Methods Sixty pathologic specimens of advanced gasrtic cancer（AGC）,twenty specimens of normal gastric mucosa（NGM）,twenty specimens of chronic gastritis of intestinal metaplasia（IM） and twenty specimens of dysplasia（DYS） were detected by immunohistochemistry with MaxVision method.Results The positive expression of ⅡA sPLA2 in IM group（80.0%）,DSY group（55.0%） and AGC group（41.7%） were higher significantly,compared with the corresponding value in NGM group（15.0%）（P0.05）.And moreover,the positive expression of ⅡA sPLA2 in AGC group was lower significantly than in IM group and DYS group gradually（P0.05）.In AGC group,the positive expression of ⅡA sPLA2 decreased remarkably in gastric carcinoma with chorion invasion or pelvic lymph node metastasis（P0.05）.Conclusion ⅡA sPLA2 may be an early event during the tumorigenesis of the gastric cancer.And moreover,the loss expression of ⅡA sPLA2 may participate in the invasion and metastasis of gastric cancer.
