HE4、CAl53联合检测在乳腺癌辅助诊断中的应用

摘　　要：目的研究血清人附睾上皮分泌蛋白4(HE4)、癌抗原153(CAl53)联合检测在乳腺癌辅助诊断中的价值。方法收集46例乳腺癌患者、52例乳腺良性疾病患者以及50名正常对照者血清,采用酶联免疫吸附试验(ELISA)检测血清HE4含量、电化学发光法检测CAl53含量。采用受试者工作特征(ROC)曲线确定HE4和CA153的临界值。分别计算HE4和CA153单项检测和联合检测的敏感性和准确性并作比较。结果乳腺癌组HE4和CAl53水平分别是49.0(41.7～67.4)pmol/L和20.6(13.4～28.7)U/mL,均明显高于乳腺良性疾病组及正常对照组(P<0.01)。经ROC曲线确定HE4和CA153的临界值分别为62.5 pmol/L和21.6 U/mL,乳腺癌HE4、CAl53联合检测的敏感性为63.0%,准确性为82.4%,均高于单项检测。结论联合检测CAl53和HE4可以明显提高乳腺癌诊断的准确性及敏感性。Objective To study the combined application value of the serum human epididymis secretary protein 4（HE4） and cancer antigen 153（CA153） in the auxiliary diagnosis of breast cancer.Methods The serum samples were collected from 46 patients with breast cancer,52 patients with benign breast disease and 50 controls.The concentrations of HE4 and CA153 were determined by enzyme-linked immunosorbent assay（ELISA） and electrochemiluminescence immunoassay respectively.The critical values of HE4 and CA153 were analyzed by receiver operating characteristic（ROC） curve.The accuracies and sensitivities of HE4 detection,CA153 detection and the combined detection of serum HE4 and CA153 were calculated and compared.Results The levels of serum HE4 and CA153 of breast cancer patients were 49.0（41.7-67.4）pmol/L and 20.6（13.4-28.7）U/mL,which were significantly higher than those of benign breast disease patients and controls（P0.01）.By ROC curve analysis,the critical values of HE4 and CA153 were 62.5 pmol/L and 21.6 U/mL,respectively.The diagnostic sensitivity and accuracy of serum HE4 and CA153 combined detection were 63.0% and 82.4%,which were higher than those of single assay.Conclusions The diagnostic sensitivity and accuracy can be elevated significantly in the patients with breast cancer when the serum CA153 and HE4 are detected simultaneously.
