摘 要:目的探讨急性脑梗死患者血浆超敏C-反应蛋白(high sensitive C-reactive protein,hsCRP)、一氧化氮(nitric oxide,NO)和一氧化氮合酶(nitric oxide synthase,NOS)水平的变化及其与病情严重程度的关系。方法分别应用乳胶增强散射比浊法和分光光度法对183例急性脑梗死患者(起病24 h内)血浆hsCRP、NO和NOS水平进行测定,并与56例健康对照和32例腔隙性脑梗死患者结果进行比较,分析急性脑梗死患者hsCRP、NO和NOS水平与神经功能损害程度的关系。患者神经功能缺失程度根据NIHSS量表评定。结果腔隙性脑梗死组血浆hsCRP水平高于对照组(P<0.05),NO和NOS水平低于对照组(P<0.05)。急性脑梗死组血浆hsCRP、NO和NOS水平均高于对照组和腔梗组(P<0.01);不同神经功能缺失组血浆hsCRP和NOS水平明显高于对照组;中度组和重度组血浆NO水平明显高于对照组,轻度组与对照组血浆NO水平差异无统计学意义;不同神经功能缺失程度各组之间血浆hsCRP、NO和NOS水平的差异有统计学意义(除中度组与重度组hsCRP比较P<0.05外,其余各组间比较P<0.01);急性脑梗死患者血浆hsCRP水平与NO及NOS水平呈明显正相关(r分别为0.848,0.833,P<0.01);血浆NO水平与NOS水平呈明显正相关(r=0.899,P<0.01)。结论 ACI患者血浆hsCRP、NO和NOS水平均显著升高,并与脑梗死严重程度密切相关,三者联合检测更有利于ACI的诊断和病情判断。Objective To investigate the changes of the plasma levels of high sensitive C-reactive protein(hsCRP),nitric oxide(NO)and nitric oxide synthase(NOS)in patients with acute cerebral infarction(ACI)and their relationships to the severity of disease.Methods The levels of plasma hsCRP,NO and NOS were compared among 183 patients with ACI(within 24 h),32 patients with lacunar infarction and 56 healthy controls.The relationships between the levels of hsCRP,NO,NOS in ACI patients and the degree of neurological damage were analyzed.The levels of plasma hsCRP,NO and NOS were measured by latex enhanced immunonephelometric assay and spectrophotometry.Results The levels of plasma hsCRP in lacunar infarction patients were higher than that in the controls(P0.05).The levels of plasma NO and NOS in lacunar infarction patients were lower than that in the controls(P0.05).The levels of plasma hsCRP,NO and NOS in ACI patients were higher than that in the controls and the lacunar infarction patients(P0.01).The levels of plasma hsCRP and NOS in the patients with severe,moderate and mild ACI were significantly higher than that in the controls(P0.01).The level of plasma NO in the severe and moderate patients were significantly higher than that in the controls(P0.01).No significant difference was found in the levels of plasma NO between the mild ACI patients and the controls(P0.05).The plasma levels of hsCRP,NO and NOS showed significant difference among the patients with different degrees of neurological damage(P0.05 or P0.01).The levels of plasma NO and NOS were positively correlated with the hsCRP(NO:r=0.848,P0.01;NOS:r=0.833,P0.01).The levels of plasma NO was positively correlated with the NOS(r=0.899,P0.01).Conclusion The levels of plasma hsCRP,NO and NOS in patients with ACI increase significantly,which may reflect the degree of cerebral infarction.Combined determination of plasma hsCRP,NO and NOS are helpful for evaluating the ACI patient's condition and prognosis.
