摘  要:目的比较伴远处转移与不伴远处转移的乳头状甲状腺癌(PTC)患者血清的蛋白质组学差异,建立伴远处转移PTC患者的血清蛋白质指纹图谱筛选模型。方法收集36例伴远处转移PTC患者和29例不伴远处转移PTC患者的血清,其中建模组45例(伴远处转移PTC26例,不伴远处转移PTC19例),盲法筛选组20例(伴远处转移PTC10例,不伴远处转移PTC10例)。采用固定金属亲和表面(IMAC3)芯片,以表面增强激光解析离子化飞行时间质谱(SELDI-TOF-MS)技术测定得到血清蛋白质谱。运用Ciphergen Proteinchip3.1软件比较伴与不伴远处转移两组PTC患者的血清蛋白质谱的差异。采用生物信息学分析方法建立决策树模型并进行盲法验证。结果在质荷比1.5~20.0ku范围内,两组患者血清中共检测到69个蛋白质峰,其中10个蛋白质峰在两组间差异具有统计学意义(P〈0.05)。用建模组数据构建的决策树模型对20个盲法筛选样本进行归类预测,其灵敏度为80%,特异度为80%。结论伴与不伴远处转移PTC患者的血清蛋白质谱存在显著差异,以此建立的决策树模型对PTC伴与不伴远处转移能够做出较为准确的预测判断。Objective To compare differentially expressed proteins between papillary thyroid carcinoma(PTC)patients with distant metastasis and PTC without distant metastasis,and establish the predictive models that may be of help to serologic diagnosis of papillary thyroid carcinoma(PTC)with distant metastasis.Methods Serum samples were collected from 36 PTC patients with distant metastasis and 29 PTC patients without distant metastasis,and randomized into a modelling set(26 PTC patients with distant metastasis and 19 PTC patients without distant metastasis)and a blinding test set(10 PTC patients with distant metastasis and 10 PTC patients without distant metastasis).Serum samples were applied to immobilized metal affinity capture(IMAC3)proteinchip surfaces and tested by surface enhanced laser desorption/ionization-time of flight-mass spectrometry(SELDI-TOF-MS).The data were analyzed by Ciphergen Proteinchip 3.1 software to screen differentially expressed proteins between PTC with distant metastasis and PTC without distant metastasis.Decision classification tree models were established by bioinformatics.Doubled-blind test was used to determine sensitivity and specificity of the classification tree models.Results A total of 69 effective protein peaks were detected at the molecular range of 1.5 ku to 20 ku,among which 10 were significantly different between PTC with distant metastasis and PTC without distant metastasis(P0.05).All the peptide data in the training set were used to obtain decision tree models.The tree with highest cross validation rate was chosen to construct the classification models which can different PTC patients with distant metastasis from PTC patients without distant metastasis.With this decision tree,16 cases were correctly forecasted from 20 blind testing samples,with a sensitivity of 80% and a specificity of 80%.Conclusions The serum protein profiles between PTC with distant metastasis and PTC without distant metastasis were differential and the predictive models can discrim
