摘 要:目的探讨肝炎肝硬化门静脉高压症胆囊壁增厚时肝动脉的血流动力学变化。方法 56例患者根据是否有门静脉高压和胆囊壁增厚分成三组:肝炎肝硬化门静脉高压症伴胆囊壁增厚(A组)、肝炎肝硬化门静脉高压症不伴胆囊壁增厚(B组)、有肝炎病史但无门静脉高压及胆囊壁增厚(C组)。彩色多普勒超声诊断仪检测肝动脉内径、收缩期峰值流速(PSV)、舒张末期流速(EDV)。结果 A组中肝动脉内径、PSV、EDV较B、C组大,差异有统计学意义(P<0.05);B组与C组比较,差异无统计学意义(P>0.05)。结论肝炎肝硬化门静脉高压症时肝动脉血流动力学改变参与胆囊壁增厚的病理生理机制。Objective To investigate the hemodynamies changes of hepatie artery(HA) in gallbladder wall thickening (GBWT) patients with portal hypertension and posthepatitic cirrhosis (PHPC) by ultrasonography. Methods Fifty -six patients were divided into three groups according to portal hypertension and GBWT. group A was PHPC with GBWT, group B was PHPC with non - GBWT, group C was non - PHPC with non - GBWT. The diameter, peak systolic velocity (PSV) and end diastolic velocity (EDV) of HA in all the patients were measured by color Doppler ultrasonography. Results Compared with group B and C, there was significant difference of diameter, PSV and EDV in group A ( P 〈 0.05 ), while there was no significant difference between group B and C. Conclusion Changes of hemodynamics of HA with GBWT in patients with PHPC are related to the inner mechanism of GBWT.
