摘  要:目的:探讨分析体部γ刀在治疗非小细胞肺癌小病灶时的剂量分布及正常组织的受量情况。方法:选取10例非小细胞肺癌患者,入组条件:病灶大小≤5 cm,10例患者分别设计体部γ刀计划和三维适形计划,分析放疗计划中PTV最小剂量、平均剂量,正常组织肺的受量情况。结果:γ刀组PTV最小剂量、最大剂量、平均剂量分别为56.6%、99.8%、76.1%,肺V20、V25、V30、V35分别为7.02%、5.11%、4.24%、3.02%;适形组PTV最小剂量、最大剂量、平均剂量分别为94.0%、104.5%、100.2%,肺V20、V25、V30、V35分别为14.08%、10.20、8.42%、7.02%。结论:针对非小细胞肺癌≤5 cm的病灶,体部γ刀在保护正常组织上优于适形放疗,可提高放疗剂量,是一种治疗局部小病灶有效且理想的放疗方法。Objective To evaluate the relationship between dose distribution and amount of normal tissue receiving dose in the treatment of body gamma knife for non-small-cell lung cancer(NSCLC).Methods 10 patients with non-small-cell lung cancer and the focus of cancer anteroposterior diameter≤5cm were adopted as the subjects.10 patients accepted body gamma knife treatment and three-dimensional conformal radiation therapy.PTV minimal dose/average dose and the receiving doses of normal pulmonic tissue were evaluated.Results The PTV minimal dose,maximum dose,average dose that patients accepted gamma knife therapy were 56.6%,99.8%,76.1% and the lung V20,V25,V30,V35 were 7.02%,5.11%,4.24%,3.02%;PTV minimal dose,maximum dose,average dose that patients accepted three-dimensional conformal radiation therapy were 94.0%,104.5%,100.2% and the lung V20,V25,V30,V35 were 14.08%,10.20,8.42%,7.02%.Conclusion For the non-smal-cell lung cancer(NSCLC) that the focus of cancer anteroposterior diameter≤5cm,gammea knife is better than three-dimensional conformal radiation therapy because it can increase radiotherapy dose.Gamma knife is an efficient and desirable therapy.
