磁共振弥散加权成像评估肝癌微波消融术疗效的临床研究

摘　　要：目的探讨磁共振弥散加权成像评估肝癌微波消融术疗效的作用。方法对35例行CT引导下微波消融术的肝癌患者,于术前1-3天、术后2天、30天行磁共振弥散加权成像检查,比较微波消融术前、后不同b值(300、500、700s/mm2)下肿瘤组织ADC值的动态变化过程。结果不同b值条件下,术前、术后的肿瘤组织平均ADC值变化有显著性差异(P<0.05),随着b值的升高,平均ADC值亦逐渐升高;术后各组的ADC平均值较术前升高,术后30天ADC平均值较术后2天平均值高。结论 ADC值与肝癌微波消融术后的肿瘤坏死密切相关,可用于评估肝癌微波消融术的疗效。Objective To evaluate the therapeutic effect of microwave ablation in advanced liver cancer patients with MR-DWI （diffusion-weighted imaging）. Methods MR-DWI examination was performed in 35 liver cancer patients on 1 to 3 days before CT-guided microwave ablation, 2 days and 30 days after microwave ablation. Then a comparison between the dynamic change process of ADC （apparent diffusion coefficient） values before and after microwave ablation under different b-values was taken. Results Under the different b value condition, the average ADC values？change of tumor organization has the significance differences among before microwave ablation and after microwave ablation （P0.05）. Along with b value＇s elevation, the average ADC value also gradually elevates .The ADC values？ change was ranged as 1-2 days before microwave ablation 2 days after microwave ablation 30 days after. Conclusion ADC value change process after microwave ablation of liver cancer is closely related to tumor necrosis, and it can be used to assess the efficacy of liver microwave ablation.
