摘  要:目的:探索超声吸引器在乳腺癌改良根治术腋窝清扫中的临床应用。方法:将瑞金医院2005年1月至2008年9月行乳腺癌改良根治术的病人随机分为超声吸引辅助清扫组(204例)与常规清扫组(191例),观察手术时间、术后引流量、放置引流管天数、术后住院天数、淋巴结清扫数目、淋巴结转移数目及术后皮下积液等并发症。结果:超声吸引辅助清扫组淋巴结清扫数目较常规清扫组显著增多(P<0.001),术后引流量、引流管放置天数、住院天数均比常规手术组显著减少(P<0.001),手术时间、淋巴结转移数目两组无统计学差异。结论:超声吸引器在腋窝中使用安全、术野清晰,淋巴结清扫较彻底,术后引流量和置管天数减少,术后皮下积液等并发症少,在乳腺癌手术腋窝清扫中有其临床应用价值。Objective To evaluate the clinical significance of using ultrasonic aspiration in axillary lymphadenectomy of modified radical mastectomy(MRM).Methods MRM were performed on 395 patients with breast cancer between Jan 2005 and Sep 2008.Patients were randomly divided into two groups: the study group included 204 operations using ultrasonic aspirator and the control group 191 operations using electrocautery only.Operation time,draining volume,draining time,hospital stay after surgery,number of dissected lymph nodes,number of metastatic lymph nodes,subcutaneous fluid accumulation and other complications were compared between the two groups.Results The number of dissected lymph nodes was significantly higher in the study group than the control group(P0.001).Draining volume,draining time and hospital stay after surgery were significantly less in the study group(all P0.001).No difference existed between the two groups regarding the operation time and the number of metastatic lymph nodes.Conclusions Our results show that the ultrasonic aspirator can be safely used in axillary lymph node dissection in MRM.By using ultrasonic aspirator in MRM,more axillary lymph nodes can be dissected,the draining time and draining volume reduced,and the hospital stay shortened.
