摘 要:目的评价自制定位引导器CT导引下对模拟目标穿刺的应用价值。方法将球体状模拟目标按直径分为0.5cm、1.0cm、1.5cm3组,每组各20个,行螺旋CT导引下穿刺术,由同一医师分别采用自制定位导引器穿刺(引导法)和常规穿刺(常规法),分为垂直和斜角45°2种进针方式,观察2种方法对各组目标的穿刺效果。结果垂直进针方式中引导法和常规法对3组不同大小模拟臼标的首次穿刺成功率分别为95.0%、100%、100%和50.0%、75.0%、95.0%,其中引导法各组间比较差异均无统计学意义,常规法0.5cm组与1.5cm组间差异有统计学意义(P〈O.01);0.5cm组2方法之间比较差异具有统计学意义(P〈0.01)。45°进针方式中2方法对3组模拟目标的首次穿刺成功率分别为95.0%、100%、100%和30.0%、55.0%、80.0%,其中引导法组间比较差异均无统计学意义,常规法0.5cm与1.5cm组间比较差异有统计学意义(P〈O.01);0.5cm、1.0cm组2方法间比较均具有统计学意义(P均〈0.01)。2种方法穿刺成功操作时间比较.引导法均低于常规法,其中0.5cm组在垂直进针及45。进针方式中2方法问差异具有统计学意义(P〈0.01)。结论采用该定位导引器可提高CT导引下对小目标的首次穿刺成功率和减少操作时间。Objective To evaluate the applied value of CT--guided puncture of mimic specimen with device made by oneself. Methods The spherical mimic models included three types,0.5 cm, l. 0 cm,1.5 cm in diameter and 20 models in each type underwent helical CT--guided puncture, the same attending doctor puncture each type specimen team 20 times, using the location and aiming device made by oneself and routine puncture, and using the different needle insertions:vertical and 45°. Then examine the results of the two methods and the two teams. Results With vertical needle insertion,the successful rates at first puncture for the three different models using guide method and routine method were 95 %, 100 %, 100 % and 50.0 %, 75.0 %, 95.0 %, respectively, there was no statistical differences among them using guide method,while there was statistical difference between 0.5 cm and 1.5 cm models using routine method(P〈0.01). The successful rate at first puncture at 45° for the three models using two methods ware 95%, 100%, 100% and 30.0% ,55.0% ,80.0% ,respectively and there were no statistical differences among them using guide method,but there were statistical differences between 0.5 cm and 1.5 cm models (P〈0.01) and between 0.5 cm and 1.0 cm models using two methods (P〈0.01, (P〈0.05). The time needed for successful puncture at the guide method was shorter than that at routine meth- od,and there was statistical difference in 0.5 cm model between the vertical and 45°insertions. Conclusion In puncture biopsy using guide machine can obviously improve the successful rate at first time and reduce the operative time under CT--guided for small target.
