疏肝和胃法治疗反流性食管炎随机对照试验的系统评价

摘　　要：系统评价中医疏肝和胃法治疗反流性食管炎的临床疗效。方法:通过计算机检索在2000年1月—2010年3月近10年期间国内公开发表的有关中医疏肝和胃法治疗反流性食管炎的临床研究文献,按照纳入和排除标准进行文献筛选,对纳入的试验作系统评价。结果:检索到相关文献83篇,最终纳入评价的文献21篇。经Meta分析显示,单纯疏肝和胃法治疗RE与西医对比,两者总有效率存在显著差异(OR=3.02,95%CI为[2.14,4.26],合并效应的检验Z=6.31,P<0.00001),单纯疏肝和胃法治疗反流性食管炎优于西药对照组;疏肝和胃法联合西药治疗RE与西医对比,两者总有效率同样存在显著差异(OR=4.20,95%CI为[1.91,9.25],合并效应的检验Z=3.57,P<0.00001),疏肝和胃法联合西药治疗反流性食管炎优于西药对照组。结论:中医疏肝和胃法治疗反流性食管炎与西药治疗相比更具有优势。Objective：To evaluate the clinical therapeutic effect of the method of soothing liver and regulating stomach on reflux esophagltis. Methods ： Correlative articles published during the period of the last decade were searched by computer. All articles were selected according to inclusion and exclusion criteria. The articles selected were evaluated by the method of systematic review. Results：there were 83 articles searched out, but only 21 articles were conformed to the inclusion and exclusion criteria. The meta analysis indicated that there was significant difference at total efficiency between treatment group using the simpte method of soothing liver and regulating stomach and comparison group using western medicine （ OR = 3.02,95% CI was [ 2. 14,4.26 ], the test of combined effect Z = 6. 31 ,P 〈 0. 00001 ）. This proved that the simple method of soothing liver and regulating stomach on reflux esophagitis is better than western medicine. Similarly, there was significant difference at total efficiency between treatment group using the method of soothing liver and regulating stomach combined with western medicine and comparison group using western medicine （ OR=4. 20,95 % CI was [ 1.91,9. 25 ] , the test of combined effect Z = 3.57, P 〈 0. 00001 ）. This proved that the method of soothing liver and regulating stomach combined with western medicine on reflux esophagitis is better than western medicine. Conclusion：Method of soothing liver and regulating stomach on reflux esophagitis is better than western medicine.
