B超引导下细针抽吸活检在判断乳腺肿块性质中的临床应用

摘　　要：通过对比研究 Ｂ超引导下的乳腺细针抽吸活检 Ｕ Ｓ Ｆ Ｎ Ａ Ｂ 及常规细针抽吸活检⒉ Ｆ Ｎ Ａ Ｂ⒕的细胞学检查结果 以探讨 Ｕ Ｓ－ Ｆ Ｎ Ａ Ｂ的临床应用价值 采用国际通用准确性统计标准统计对７１ 例 Ｕ Ｓ－ Ｆ Ｎ Ａ Ｂ病例及９２ 例 Ｆ Ｎ Ａ Ｂ病例结果进行分析 结果 Ｕ Ｓ－ Ｆ Ｎ Ａ Ｂ对乳腺肿块诊断的灵敏度为６３％ 特异性为７３％ ⒙阳性准确率为６８％ ⒙阳性预测值为１００％ ⒙阴性预测值为７３％ 均高于 Ｆ Ｎ Ａ Ｂ组⒚而 Ｕ Ｓ－ Ｆ Ｎ Ａ Ｂ组取材无结构的发生率为１４％ 低于 Ｆ Ｎ Ａ Ｂ组 同时对于直径≤２ｃｍ 的肿块⒙ Ｕ Ｓ－ Ｆ Ｎ Ａ Ｂ组的灵敏度及准确性均显著高于 Ｆ Ｎ Ａ Ｂ组 结论⑶ Ｕ Ｓ－ Ｆ Ｎ Ａ Ｂ可提高针刺活检对乳腺肿块的诊断效率Objective:To investigate the clinical value of US FNAB by comparing the cytological results with FNAB.Methods:71 US-FNAB cases and 92 FNAB cases were analyzed with the international statistics standard.Results:US FNAB is superior to FNAB,the sensibility,specificity,positive accuracy,positive predict value and negative predict value is 63%,73% 68%,100% and 73% respectively.The incidence rate of no structure in US FNAB is 14%,inferior to FNAB.To cases of diameter≤2cm,the sensitivity and accuracy in US-FNAB is significant to that in FNAB.Conclusion:the method of US-FNAB can increase the efficiency of aspiration biopsy to breast masses,it has better practical value in clinic.
