摘 要:目的微波固化范围能否覆盖整个目标肿瘤是其能否取得根治性疗效的关键,本研究探讨经皮水冷式微波固化对于提高其固化范围的作用及其在肝肿瘤治疗中的作用。方法从2009年7月至2010年12月,58例肝肿瘤患者(49例原发性肝癌,9例转移性肝癌,肿瘤最大直径≤5cm且肿瘤数目≤3)接受经皮水冷式微波固化术,探讨其固化范围及其疗效。结果58例患者共96个肿瘤[肿瘤直径介于1.2~4.8cm,平均直径为(3.8±1.1)cm)]接受微波水冷式微波固化术,水冷式微波固化范围为(5.2±1.4)cm(固化范围介于3.6~6.3cm),共89个肿瘤取得完全消融,完全消融率为92.7%。全组患者无严重并发症发生.2例患者术后出现右侧胸腔少量积液。结论水冷式微渡固化术能产生较大固化范围,对于直径≤5cm肝肿瘤基本能达到完全固化的目的.是安全的微创治疗手段。Objective The coagulation zone achieved by microwave which is large enough to cover the target tumor is the key to achieve radical treatment effect. This study is to explore the efficacy of cool-tip microwave to improve the coagulation and its application for liver malignant tumor. Methods From July 2009 to December 2010, 58 patients with liver malignant tumor (49 primary and9 metastases) were treated by percutaneous cool-tip microwave. The efficacy was assessed. Results 58 patients with total 96 tumors [(1.2 - 4.8 cm, ayerage diameter (3.8 ± 1.1) cm] were treated by percutaneous cool-tip microwave coagulation. The average diameter of coagulation zone was (5.2 ± 1.4) cm (3.6 - 6.3 cm). 89 tumors achieved complete coagulation and the complete coagulation rate was 92.7%. No major complication happened according to the procedure, and hydrothorax happened in 2 patients. Conclusions As a safe mini-invasive treatment, pereutaneous cool-tip microwave can induce larger coagulation zone to achieve radical efficacy for liver malignant tumor less than 5 cm in diameter.
