胃平滑肌肿瘤的螺旋CT诊断

摘　　要：目的: 探讨胃平滑肌肿瘤的螺旋CT 表现、鉴别诊断及检查技术。材料与方法: 采用充分口服温开水( 或0 .5 % ～1 .0 % 的泛影葡胺) 、低张和及时扫描的CT 检查方法,分析21 例经手术病理证实胃平滑肌肿瘤的螺旋CT 表现。结果: 12 例平滑肌瘤CT 表现多为圆形或椭圆形软组织肿块,与胃壁广基底或带蒂相连,直径多在5cm 以内。除瘤灶内有散在钙化或中央呈低密度各1 例外,其余肿块密度均匀,强化均一。9 例平滑肌肉瘤CT 表现为椭圆形或不规则肿块,宽基底与胃壁相连,内见不规则低密度坏死区,实质部分不均匀强化,胃粘膜面多有大小不等的溃疡,肿块直径多大于5cm 。有1 例广泛侵及大、小网膜、脾曲结肠及肝门区。结论: 螺旋CT 扫描对胃平滑肌肿瘤诊断及鉴别诊断颇有价值。Objective: To investigate spiral CT manifestations, distinguishing and examining techniques for gastric smooth muscle tumors.Materials and Methods:Technically, sufficient oral contrast agents (warm water or 0.5%～1.0% diatrizoate), hypotonic medicine and prompt CT scanning were required. Spiral CT findings of 21 cases with pathologically proved gastric smooth muscle tumors were analyzed.Results:Round or ovoid soft tissue mass with wide fundus or narrow pedicle adhering to gastric wall was seen in 12 cases with gastric leiomyoma. Most lesions were less than 5cm in diameter. Disseminated calcifications and central low density area were found in one case each. In 9 cases of leiomyosarcoma, heterogeneous ovoid and irregular soft tissue mass with wide fundus adhering to gastric wall, carrying low density necrotic areas and different sized mucosal ulcers was seen. Most lesions were larger than 5cm in diameter. Extensive involvement of greater and lesser omentum, splenic curvature of the colon and hepatic portal was found in one case.Conclusion: Spiral CT is valuable in the diagnosing and differentiating of gastric smooth muscle tumors, and is especially superior to barium meal exam and endoscopy for exophytic smooth muscle tumors of the stomach.
