摘  要:目的探讨大鼠慢性肺动脉高压模型下腔静脉多普勒血流频谱的改变及其意义。方法采用野百合碱腹腔注射建立SD大鼠慢性肺动脉高压模型,肺动脉高压模型组32只,对照组8只(注射相同剂量0.9%氯化钠无水乙醇溶液)。采用超声多普勒技术观察记录模型建立前后下腔静脉多普勒血流频谱变化,行右心导管测定肺动脉压力。结果成功建立慢性肺动脉高压大鼠模型28只,肺动脉高压模型组大鼠肺动脉收缩压、舒张压、平均压、右心室收缩压均明显高于对照组(P<0.05)。给药4周后大鼠右心室及右心房上下径增大,与给药前相比差异有统计学意义(P<0.05)。肺动脉高压模型组给药4周后下腔静脉回心血流速度峰值(S)较给药前显著降低(P﹤0.01),4周后的离心血流速度峰值(a)较给药前显著增高(P<0.01),回心血流峰值与离心血流峰值的比值(S/a)降低,与给药前相比,差异具有统计学意义(3.027±1.094,5.768±2.545,P﹤0.01)。结论下腔静脉血流频谱变化可以反映肺动脉压力变化,下腔静脉回心血流峰值与离心血流峰值的比值可以为肺动脉压力评估提供有效参考指标。Objective To establish the rat model with chronic pulmonary hypertension and assess the significance of the changes for the inferior vena cava Doppler flow spectrum.Methods Forty SD rats were randomly divided into to groups:the control group including 8 rats and the chronic pulmonary hypertension group including 32 rats,which were administered in the same condition.The chronic pulmonary hypertension in a rat model was developed by monocrotaline solution injecting into abdominal cavity,and the control group was then administered into abdominal cavity by the same dosage of the mixed liquor of pure ethanol and physiologic saline.The inferior vena cava Doppler flow spectrum of SD rat model with chronic pulmonary hypertension was monitored by echocardiography.The pulmonary artery pressure of right heart catheterization and the changes of inferior vena cava Doppler flow before and after the establishing of the model were simultaneously recorded.Results Twenty eight rat models with chronic pulmonary hypertension were successfully established.The pulmonary artery systolic pressure,pulmonary artery diastolic pressure,mean pulmonary artery pressure and the right ventricular systolic pressure were significantly higher(P0.05) than those of the control group.Four weeks after injection,the upper and lower diameter of right ventricle and atrium increased significantly compared with those before injection(P0.05).The inferior vena cava blood flow velocity(S) in four-week model group was significantly decreased than that before injection(P﹤0.01).And the reverse flow velocity(a) in four weeks group was significantly increased than before injection(P0.05).The ratio of blood flow velocity and the reverse flow velocity(S/a) significantly decreased compared with the ratio of those before injection(P0.05).Conclusion The changes of inferior vena cava flow spectrum can be helpful for diagnosis of pulmonary hypertension.The ratio of blood flow velocity and the reverse flow velocity in inferior vena cava
