超声造影观察肝脏局灶性结节样增生小病灶周边的粗大滋养动脉的临床价值

摘　　要：目的探讨超声造影观察肝脏局灶性结节样增生(FNH)小病灶(最大直径<3cm)周边的粗大滋养动脉的临床价值。方法超声造影观察25例FNH患者的25个小病灶(最大直径<3cm),在动脉期对病灶增强模式和病灶周边的粗大滋养动脉进行分析,同时随机选取20个肝血管瘤及20个肝癌小病灶(直径均<3cm)与之对照。结果 25个FNH小病灶中18个病灶(72.0%)于动脉早期呈轮辐状离心性强化,其他病灶则均为全瘤迅速强化。在FNH组、肝血管瘤组、肝癌组中分别有19个(76.0%)、1个(5.0%)、11个(55.0%)病灶于病灶周边检出了粗大滋养动脉。结论 FNH小病灶周边的粗大滋养动脉检出率较高,有助于FNH的定性诊断。Objective To investigate the the dilated feeding arteries sign around small focal nodular hyperplasia（FNH）（ 3 cm,in diameter） with contrast-enhanced ultrasound（CEUS）.Methods A total of 25 FNH patients with 25 lesions took examination of CEUS to observe the dilated feeding arteries sign in arterial phase.Meanwhile,20 patients with liver hemangiomas and 20 patients with hepatocellular carcinoma（ 3 cm,in diameter） had the same examination.Results The number of the lesions with spoke-wheel arteries,centrifugal filling in arterial phase were 18 in 25 small lesions（72.0%,18/25）,other lesions showed completely hyper-enhancing in arterial phase.The dilated feeding arteries around FNH,hemangiomas,hepatocellular carcinoma,in arterial phase were exhibited in 19 lesions（76.0%）,1 lesion（5.0%）,11 lesions（55.0%）,respectively.Conclusion The dilated feeding arteries sign in arterial phase was easy to be observed in small FNH,which may be helpful for diagnosis of FNH.
