动脉粥样硬化性脑梗死患者MMP-2、3、9动态变化及与颈部斑块关系研究

摘　　要：目的 研究基质金属蛋白酶（MMP）2、3、9血清水平与动脉粥样硬化性脑梗死的关系.方法 应用ELISA测定82例动脉粥样硬化性脑梗死患者发病24h内、第3天以及40名健康对照者血清MMP-2、3、9的水平;彩色血管超声检查患者颈部血管,根据斑块性质分为不稳定斑块组、稳定斑块组及无斑块组3个亚组进行比较.结果 脑梗死发病24h内、3d血清MMP-2、3、9逐步增高.与对照纽比较.差异均有显著性（P〈0.01）,MMP-9的活性改变最大.发病24h内,脑梗死亚组MMP-2,MMP-3和MMP-9水平有差异,和稳定斑块组、无斑块组比较不稳定斑块组水平都明显升高,差异有显著性（P〈0.01）.结论 血清MMP-2、3、9水平与动脉粥样硬化性脑梗死发病有关,MMP-2、3、9是缺血后继发性脑损伤的重要原因,尤其是MMP-9.MMP-2、3,9血清水平与颈动脉斑块稳定性有关.Objeetive To investigate the association of matrix metallproteinase-2,3,9 concentration to atherosclerotic cerebral infarction.Method The study enrolled 82 atherosclerotic cerebral infarction patients （collected 24 hours and 3 days after the onset）and 40 healthy controls.Serum concentration of MMPs was measured by enzyme linked immunoabsorbent assay（ELISA）.The atherosclerotic plaques in the carotid arteries were assessed by Color Doppler Ultrasonography.Patient group was divided into three subgroups according to the characteristic of atherosclerosis：unstable plaque group,stable plaque and no plaque group.Result The serum level of MMP-2,3,9 in the patients was significantly higher either in 24 hours or 3 days after the onset than the healthy controls;Compared with the healthy controls,there was significant difference in them.The serum levels of MMP- 2,3,9 in 24 days after the onset in unstable plaque group was significantly higher than stable plaque group and no plaque group. Conclusion There may be a close relation between serum level of MMP-2,3,9 with pathogeny of atherosclerotic cerebral infarction.MMP-2,3,9,especially MMP-9,play important roles in ischemic brain injury.The concentration of serum MMP-2,3,9 has a positive relation to the carotid atheroselerosis.
