不同胃黏膜病变中TFF3表达与幽门螺杆菌的关系

摘　　要：目的观察三叶因子3(trefoil factor 3,TFF3)在胃癌前病变及胃癌的表达,及其与幽门螺杆菌(Helicobacter pylori,H.pylori)感染的相关性,探讨H.pylori感染与胃癌发生的关系。方法收集慢性浅表性胃炎(chronic superficial gastritis,CSG),慢性萎缩性胃炎伴中、重度肠上皮化生(intestinal metaplasia,IM),慢性萎缩性胃炎伴中、重度不典型增生(dysplasia,Dys)各30例,胃癌(gastric cancer,GC)40例,采用免疫组织化学SP法检测TFF3蛋白的表达。采用warthin-starry银染法及尿素酶试验检测胃黏膜H.pylori感染。结果 CSG、IM、Dys、GC组织中H.pylori感染的胃黏膜组织中,TFF3的表达高于无H.pylori感染的患者(P<0.05)。结论 H.pylori感染可能通过上调TFF3的表达,在胃癌发生、发展过程中发挥重要作用。Objective To explore the relationship of trefoil factor 3 （TFF3） and helicobacter pylori（ H. priori ） infection in gastric cancer and precancerous lesion, and discuss the relationship between H. pylor infection and gastric cancer. Methods The expression of TFF3 was determined by immunohistochemical method in 130 gastric specimens including 30 cases of chronic superficial gastritis, 30 cases of intestinal metaplasia,30 cases of dysplasia and 40 cases of gastric carcinomas. The H. priori was detected by warthin - starry method. Results In control group with chronic superficial gastritis, intestinal metaplasia group,dysplasia group and gastric carcinoma group,the expression levels of TFF3 had an increasing tendency （ P 〈 0.05）. The expression of TFF3 in H. pylori positive group were significantly higher than that in negative group （ P 〈 0.05）. Conclusion H. priori infection may play a key role in the carcinogenesis and development of gastric carcinoma by up - regulating the expression of TFF3.
