CK19、Ki67及VEGF在甲状腺乳头状癌中的表达及临床意义

摘　　要：目的探讨CK19、Ki67及VEGF在甲状腺乳头状癌表达状态及其临床价值。方法运用免疫组织化学MaxVisonTM法检测30例甲状腺乳头状癌、20例良性乳头状增生、20例甲状腺腺瘤中CK19、Ki67及VEGF的表达。结果 CK19、Ki67及VEGF在甲状腺乳头状癌阳性表达率分别为83.3%、66.7%及73.3%,3种抗体在甲状腺乳头状癌和良性病变中的阳性表达差异均有统计学意义。CK19、Ki67及VEGF诊断乳头状癌的灵敏度分别为83.3%、66.6%及73.3%,特异度分别为80.0%、77.5%及75.0%;ROC曲线下面积分别为0.868、0.752及0.781。结论 CK19、Ki67及VEGF在甲状腺乳头状癌中表达增强,在病理诊断中有一定的价值。Objective To study the expression of CK19,Ki67 and VEGF in papillary thyroid carcinoma（PTC） and their diagnostic values.Methods Expression of CK19,Ki67 and VEGF was examined by immunocytochemical staining in 30 cases of PTC,20 cases of papillary thyroid hyperplasia and 20 cases of thyroid adenomas.Results The positive rates of CK19,Ki67 and VEGF expression in papillary thyroid carcinoma were 83.3%,66.7% and 73.3%,respectively,which were significantly higher than those in benign lesions.The area under the ROC curve（AUROC） for CK19,Ki67 and VEGF were 0.868,0.752 and 0.781,respectively.The diagnostic sensitivity and specificity were 83.3% and 80.0% for CK19,66.6% and 77.5% for Ki67,and 73.3% and 75.0% for VEGF,respectively.Conclusion The expression levels of CK19,Ki67 and VEGF are significantly higher in papillary thyroid carcinoma than those in benign thyroid tissue.The detection of CK19,Ki67 and VEGF expression can be useful in the diagnosis of papillary thyroid carcinoma.
