宫颈癌先期化疗的临床应用

摘　　要：目的：研究宫颈癌先期化疗的有效率,与传统直接手术比较病理及预后的改善。方法：选择Ⅰb2~Ⅱb期宫颈癌患者52例,把52例患者分为2组：研究组（先期化疗组）,对照组（直接手术组）。研究组予先期化疗1~3疗程后评估,有效者行广泛子宫切除＋盆腔淋巴结清扫术,无效者改行根治性放疗;对照组直接行手术。总结先期化疗的有效率;比较2组的手术切除率,淋巴结转移率,宫旁转移率,脉管转移率,3年生存率。结果：研究组先期化疗的有效率为82.4%;研究组与对照组比较,手术切除率,淋巴结转移率、宫旁转移率、3年生存率有显著差异;脉管浸润率比较无显著差异。结论：先期化疗用于局部晚期宫颈癌有效,可显著提高手术切除率,减少预后不良因素,改善患者预后,提高术后生存率。Objective：To study the effective rate of neoadjuvant chemotherapy for cervical cancer and the improvement of pathology and prognosis when compared with the traditional operation without chemotherapy.Methods：Total 52 inpatients with stage Ⅰb2 and Ⅱb cervical cancer were chosed and divided into two groups.The two groups were the study group（neoadjuvant chemotherapy group） and the control group（direct operation group）.The study group were evaluated after treated with neoadjuvant chemotherapy for one to three cycles.The improved patients were treated with radical hysterectomy and pelvic lymphadenectomy,while the non-improved ones were treated with radical radiotherapy.The control group were treated with operations without chemotherapy.Totaled the effective rate of neoadjuvant chemotherapy and compaired the rates of operation,lymp node metastasis,para-uterus infiltration,vascular metastasis and 3-year survival.Results：In study group,the total effective rate of neoadjuvant chemotherapy was 82.4%;There were significant differences in the rates of operation,lymp node metastasis,para-uterus metastasis and 3-year servival between the two groups,but there was no obvious difference in the rate of vascular metastasis.Conclusion：Neoadjuvant chemotherapy is an effective treatment of advanced cervical cancer,which can obviously improve the operating rate,the survival probability and the patient＇s prognosis.Besides,factors of bad prognosis can be reduced.
