多层螺旋CT导引下孤立性肺结节穿刺活检分析

摘　　要：目的探讨多层螺旋CT(MSCT)导引下半自动活检针经皮肺穿刺活检对孤立性肺小结节的诊断价值。方法选取50例孤立性肺结节患者,均行CT导引下经皮肺穿刺活检,对所取组织行组织病理学检查。以术后病理及临床和影像结果为最终诊断,计算经皮肺穿刺活检的阳性率、成功率以及并发症发生率。结果 MSCT导引下肺穿刺活检病理检查结果阳性率为96%(48/50),穿刺成功率为98%(49/50)。并发症:气胸11例(22%),出血13例(26%),气胸合并出血5例(10%),咯血1例(2%),均较轻微,无严重并发症。结论 MSCT导引下半自动活检针经皮肺穿刺活检是一种安全、快捷、准确的诊断孤立性肺结节的方法。Objective To explore the value of multi-slice spiral computed tomography（MSCT）-guided semi-automatic cutting needle for the biopsy of solitary pulmonary nodule. Methods Totally 50 patients with solitary pulmonary nodule underwent percutaneous biopsy under the guidance of MSCT.The obtained tissues were histopathologically examined.With the post-operative pathological test results and the clinical and imaging findings as the golden standards,the postive rate,success rate,and incidence of complications of the percutaneous biopsy were calculated. Results The positive rate was 96%（48/50） and the puncture success rate was 98%（49/50） for the percutaneous biopsy.The complications included pneumothorax（n=11）,hemothorax（n=13）,pneumothorax combined with hemothorax（n=5）,and emptysis（n=1）.All the complications were mild,and no severe complication was noted. Conclusion MSCT-guided semi-automatic cutting needle for the biopsy of lung is safe,fast,and accurate,and can be used for the diagnosis of solitary pulmonary nodule.
