小脑蚓部肿瘤的CT诊断(附95例分析)

摘　　要：目的： 提高小脑蚓部各肿瘤ＣＴ 诊断的正确性。材料与方法： ９５ 例经手术、病理证实的小脑蚓部肿瘤，男性５９ 例，女性３６例，术前均行ＣＴ 平扫及增强扫描。结果： 成神经管细胞瘤４１ 例（ 男∶女＝ ２１∶２０ ，平均年龄１７ ．４ 岁） ，ＣＴ 平扫以略高、等或略低混杂密度为主，增强后肿瘤可均匀或不均匀强化。星形胶质瘤２９ 例（ 男∶女＝ １９∶１０ ，平均２０ ．８ 岁） ，ＣＴ 平扫以略低、低混杂密度为主，增强后可轻度、不规则强化。室管膜瘤１０ 例（ 男∶女＝ ８∶２ ，平均２３ 岁） ，平扫以等、混合密度为主，增强多有均匀强化，常伴脑积水。成血管细胞瘤９ 例（ 男∶女＝ ５∶４ ，平均４０ 岁） ，平扫可分实质性和囊变两大类，增强后实质性肿瘤呈明显均匀强化，囊变者为大囊及附壁明显强化的小结节。另外少枝胶质瘤１ 例，转移瘤３ 例，脉络丛乳头状瘤１ 例，海绵状血管瘤１ 例。结论： 根据患者的年龄及ＣＴ表现有助于小脑蚓部肿瘤的鉴别诊断。Objective: To improve CT diagnostic accuracy for cerebellar vermis tumors.Materials and Methods:Plain and enhanced CT scanning were performed in 95 patients with surgically and pathologically proved cerebellar vermis tumors.Results:Medulloblastoma (n=41) mostly demonstrated slightly hyperdensity or inhomogenous density on plain CT scan, and showed homogenous or inhomogenous enhancement after contrast injection. Astrocytoma (n=29) was predominantly inhomogenously low density on plain CT scan and inhomogenously enhanced after contrasted. Ependymoma (n=10) was mainly iso or inhomogenous in density on plain CT scan and homogenously enhanced on post contrasted CT scan. All cases with ependymoma accompanied hydrocephalus. Hemangioblastoma (n=9) could be cystic mass (n=7) or solid mass (n=2) on plain CT scan. On post contrasted scan, the cystic hemangioblastoma showed a large cyst with marked enhanced nodules on the wall, while the solid lesion revealed significant homogenous enhancement and enlarged drain veins. Besides, oligodendroglioma (n=1), metastases (n=3), papilloma of chorial plexus (n=1) and cavernous hemangioma (n=1) were found in this study.Conclusion: Patient's age and CT findings are helpful in making differential diagnosis for cerebellar vermis tumors.
