摘  要:目的通过对我院10年间胃镜诊断的反流性食管炎(reflux esophagitis,RE)病例分析,探讨高原地区RE的临床特征。方法收集我院2000年1月~2010年5月经胃镜检查诊断为RE患者930例,根据1999年烟台全国反流性食管病/炎研讨会制定的标准分级,对其年龄分布、临床症状及伴发疾病、幽门螺旋杆菌感染率资料进行回顾性分析。结果 10年间RE占总胃镜检查人数的7.77%,其中轻中度者92.69%,重度者仅7.31%;有反流症状者占69.78%,反流症状的发生率在轻、重度患者间中无显著差异,但中青年男性、合并消化性溃疡等在重度RE中的比例明显较高。RE的幽门螺旋杆菌感染率为78.12%;主要伴发疾病为浅表性胃炎、十二指肠溃疡。男女比例为3.41∶1。结论 RE在高原地区的发病率高于平原地区,在中青年人群中好发,仍以轻度RE为主,RE的程度不随年龄的增长而加重。Objective To explore the clinical features of reflux esophagitis(RE)in high-altitude areas.Methods The clinical data of 930 cases of RE diagnosed by gastroscopy in our hospital from January 2000 to May 2010 were collected.They were graded according to the criteria formulated during Yantai national symposium of esophageal reflux disease/inflammation in 1999.The age distribution,clinical symptoms,concomitant diseases and the infection rate of helicobacter pylori were retrospectively analyzed.Results 7.77% of 930 patients receiving gastroscopy had RE.Among them,the mild and moderate accounted for 92.69%,and the severe only accounted for 7.31%.69.78% of RE patients had reflux-related symptoms,and the incidence of these symptoms showed no significant differences among the mild,moderate and severe.However,the proportions of young and middle-aged men as well as the occurrence of peptic ulcer were apparently higher in severe RE.The infection rate of Helicobacter pylori in RE patients was 78.12%.Main concomitant diseases included superficial gastritis and duodenal ulcer.The ratio of male to female was 3.41∶ 1.Conclusion The incidence of RE in high-altitude areas is higher than that in plain region.It prevails among young and middle-aged men,and the mild is predominant.Moreover,the degree of RE does not aggravated along with age.
