超声内镜对浸润型胃癌的诊断价值

摘　　要：目的:探讨超声内镜对浸润型胃癌的诊断价值。方法:分析38例浸润型胃癌的内镜结果,对其中22例行超声内镜检查,并分析其超声影像特征。结果:浸润型胃癌活检阳性率为52.63%。超声内镜显示病变胃壁弥漫性增厚,以2、3、4层为主,呈低回声改变,多数胃壁层次尚可见,部分仅见4层结构,超声内镜诊断准确率为95.45%。行根治性手术和姑息手术者具有不同的超声特点。结论:超声内镜能提高内镜诊断浸润型胃癌的准确率,并对其可切除性的判断有重要价值。Objective：To evaluate the diagnostic value of endoscopic ultrasonography（EUS） in infiltrative gastric carcinoma.Methods： Thirty-eight cases with infiltrative gastric carcinoma were examined by gastric endoscope.Twenty-two of the patients were referred for EUS evaluation.EUS images of infiltrative gastric carcinoma were analysed.Results： The positive biopsy rate of infiltrative gastric carcinoma was 52.63%,in all patients with a final diagnosis,and EUS showed diffusive thickening of layers,especially,of the 2nd,3th and 4th layers with the lesion site,the lesion showed hypoechoic patterns.Gastric wall could be seen mostly,partly only had fourth layer.Diagnostic accuracy rate was 95.45% by EUS.The cases,performed with curative surgery or palliative operation,have different ultrasound characteristics.Conclusion： EUS can raise the diagnostic accuracy rate obviously and also have the important value of predicting the feasibility of surgical operation.
