基质金属蛋白酶及其组织抑制剂和CD44v6在子宫内膜癌中的表达及意义

摘　　要：目的:研究基质金属蛋白酶(matrix metalloproteinase,MMP)-2、MMP-9及其组织抑制剂(tissue inhibitor of metalloproteinase,TIMP)-1、TIMP-2和细胞黏附分子CD44v6在子宫内膜癌中的表达及临床意义。方法:采用免疫组织化学方法分别检测73例子宫内膜癌组织和27例正常增生期子宫内膜组织MMP-2、MMP-9、TIMP-1、TIMP-2和CD44v6蛋白的表达情况。根据子宫内膜癌的病理分级、组织学类型、淋巴结转移和肌层浸润程度不同对内膜癌进行组内分组比较。结果:内膜癌组织中MMP-2、MMP-9、TIMP-1、TIMP-2和CD44v6的表达率分别为75.3%、83.6%、75.3%、72.6%和74.0%,均高于对照组的22.2%、28.6%、57.1%、18.5%和45.0%(P<0.05)。其中,MMP-2、TIMP-2和CD44v6随肌层浸润程度的加深而表达增强;MMP-9蛋白在组织分级低、有肌层浸润和淋巴结转移的子宫内膜癌组织中表达增强。相关分析显示,MMP-2和TIMP-2、MMP-2和CD44v6有明显的正相关性。结论:MMP-2、MMP-9、TIMP-1、TIMP-2和CD44v6在子宫内膜癌的发生、发展、浸润和转移中起到一定的作用;联合测定这些指标可为子宫内膜癌的早期临床诊断、预后判断和治疗随访提供指导作用。Objective： To study the expression and clinical significance of matrix metalloproteinase（MMP）-2,MMP-9;tissue inhibitor of metalloproteinase（TIMP）-1,TIMP-2 and cellular adhesive molecule CD44v6 in the endometrial carcinoma.Methods： Seventy-three cases of endometrial carcinoma and 27 cases of normal hyperplasia endometrium were included.The protein expression of MMP-2,MMP-9,TIMP-1,TIMP-2 and CD44v6 were evaluated by immunohistochemistry.Comparisons between pathological grade,histologi-cal type,lymph nodes metastasis and invasion of uterine muscular layer were performed.Results： That the rates of expression of MMP-2,MMP-9,TIMP-1,TIMP-2 and CD44v6 were 75.3%,83.6%,74.0%,75.3% and 74.0%,were all significantly higher than that in the normal hyperplasia endometrium（22.2%,28.6%,57.1%,18.5% and 45.0%）（P 0.05）.The expression of MMP-2,TIMP-2 and CD44v6 in-creased with the invasion of uterine muscular layer,and the level of MMP-9 increased in the poor differentiated,muscular invasive and lymph nodes metastic endometrial carcinoma.In addition,there were signifi-cantly positive correlations between MMP-2 and TIMP-2,MMP-2 and CD44v6.Conclusion：MMP-2,MMP-9,TIMP-1,TIMP-2 and CD44v6 have an important role in the occurrence,development,infiltration and metastasis of endometrial carcinoma.Combining evaluation of these markers can be helpful for early clinical diagnosis,prognosis,treatment and follow up in the endometrial carcinoma.
