妇科恶性肿瘤合并多原发癌19例临床分析

摘　　要：目的提高对妇科恶性肿瘤合并多原发癌的认识。方法对19例妇科恶性肿瘤合并多原发癌的临床资料进行回顾性分析。结果异时性恶性肿瘤9例,第一原发癌发生年龄25-52岁,中位年龄41岁,两癌发生中位间隔时间为5年,发生部位在子宫、乳腺、输尿管、卵巢、结肠、甲状腺。同时性多原发恶性肿瘤10例,发病年龄19～68岁,中位年龄51.5岁,发生部位在卵巢、子宫、宫颈、胃、肺。本组多原发癌的治疗,行手术治疗,术后采用相应的化疗和(或)放疗。随访的14例中,仍存活8例,另6例第2原发癌后的生存时间为1.5～6年。结论重视妇科恶性肿瘤合并多原发癌,对有高危因素的恶性肿瘤患者应进行定期、全面检查。Objective To study metachronous or synchronous multiple primary malignant tumors in patients with gynecological malignant tumor. Methods The clinical data of 19 patients with gynecological malignant tumor were analyzed retrospectively. Results Metachronous multiple primary malignant tumors were found in 9 patients, the age of the first primary malignant tumor was 25 - 52 years, the median age was 41, and the median interval between the first and the second primary malignant tumors was5 years. The primary malignant tumors were located at uterus, breast, ureter, ovary, colons, and thyroid gland. Synchronous multiple primary malignant tumors were found in l0 patients with 19 -68 years of age, the median age was 51.5 years. It was located at ovary, uterus, cervix uteri, ventriculus, and lung. All patients were treated by surgery, chemotherapy and （or） radiotherapy. Fourteen patients were followed up,8 of 14 patients are alive,the survival time of the other 6 second malignant patients were 1.5 - 6 years. Conclusion The patients of multiple primary malignant tumors with gynecological malignant tumor need more recognition and the high - risk patients of malignant tumor should be followed up and examined routinely as well as thoroughly.
