不同临床分型肝豆状核变性患者血浆CCK、VIP和SS水平与胆囊收缩功能关系的探讨

摘　　要：目的:探讨不同临床分型肝豆状核变性(HLD)患者血浆胆囊收缩素(CCK)、血管活性肠肽(VIP)和生长抑素(SS)水平与胆囊收缩功能的关系。方法:选择符合诊断标准的HLD患者37例,其中脑型18例,肝型9例,混合型10例。18位健康人为对照组。采用放射免疫法检测两组血浆CCK、VIP、SS水平,B超测定脂餐前面积(BS)和餐后2h胆囊面积(AS),计算胆囊收缩率(GCR)。结果:(1)各临床分型组HLD患者CCK水平均显著高于对照组(P<0.05),肝型及混合型组VIP和SS水平显著高于对照组和脑型组。(2)肝型和混合型组BS和AS显著大于对照组和脑型组(P<0.05),脑型组与对照组差异无显著性(P>0.05);HLD患者GCR均显著小于对照组(P<0.05)。结论:不同临床分型HLD患者CCK、VIP和SS水平不同,并且对胆囊收缩功能的影响不同。Objective To investigate the relationship between plasma cholecystokinin （CCK）, vasoactive intestinal peptide （VIP）, somatostatin （SS） and the contractile function of gallbladder in hepatolenticular degeneration （HLD） patients of different clinical types. Methods Plasma CCK, VIP and SS from 37 HLD patients and 18 healthy persons were tested by radioimmunoassay （RIA）.The surface areas of gallbladder of HLD patients and healthy persons were measured by B-Ultrasound before and after fat diets, and gallbladder contraction rates were calculated. 37 HLD patients were divied into three groups, cerebral group （n = 18）, hepatic group （n = 9） and mixed group （n = 10） according to their main clinical manifestations. Restults CCK in HLD patients was significantly higher than those in healthy controls （P 〈 0.05 ）, and VIP and SS in hepatic and mixed group were significantly higher than those in healthy controls and cerebral group. The surface areas of gallbladder before and after fat diets in hepatic and mixed group were significantly larger than those in healthy controls and cerebral group （P 〈 0.05）, but there was no significant difference between cerebral group and healthy controls （P 〉 0.05）. The gallbladder contraction rates in HLD patients were significantly less than those in control group （P 〈 0.05）. Conclusion Plasma CCK,VIP and SS play different roles in the gallbladder contractile function of HLD patients of different clinical types.
