胸大肌锁骨部蒂半片锁骨瓣修复锁骨肩峰端的应用解剖

摘　　要：目的为锁骨肩峰端骨不连修复提供新的手术方法。方法在４０侧成人尸体标本上，观察了胸大肌锁骨部的形态、血供及其与锁骨的关系。结果锁骨部长度为１２．８ｃｍ；血供主要来自胸肩峰动脉三角肌支，其外径１．９ｍｍ、长度为４．７ｃｍ，进入三角肌前，发出１～３支、外径０．５～０．８ｍｍ的肌支进入锁骨部，肌支在肌内沿肌束走行和分支，并向上参与锁骨骨膜血管的形成。结论可设计以胸大肌锁骨部为蒂半片锁骨瓣转位修复锁骨肩峰端骨不连的新术式。Objective In order to provide new operation for repairing non unions of the clavicular acromial end Methods The form,vessels and clavicular relation of the clavicular part of greater pectoral muscle were observed on 40 sides of adult cadaveric Results The length of clavicular part was is 12 8cm The supply of blood is deltoid branch of thoracoacromal artery with diameter 1 9mm length 4 7cm,before enter deltoid send off 1～3 branches with diameter 0 5～0 8mm into clavicular part,the muscle branch go and branch along fascicle in muscle,and upwards form vessels of periosteum Conclusion It may devise a new operation for repairing non unions of the clavicular acromial end with partial clavicular bone flap pedicled with the clavicular part of greater pectoral muscle
