摘　　要：目的探讨DR-nm23基因及其蛋白在大肠癌组织中的表达及意义。方法应用核酸原位杂交和SP免疫组化方法,从mRNA和蛋白两个水平上检测98例大肠癌、57例大肠腺瘤及42例正常大肠黏膜组织中DR-nm23的表达情况。结果 DR-nm23 mRNA在正常组、大肠癌组和腺瘤组中的表达水平逐渐下降,3组间比较差异显著(P<0.01);腺瘤组中,高级别上皮内肿瘤其表达率低于低级别上皮内肿瘤,但表达差异不显著(P>0.05);DR-nm23 mRNA表达与淋巴结转移呈负相关(P<0.01),与肿瘤病理分级相关(P<0.01),但与组织学分型无关(P>0.05)。DR-nm23蛋白在正常组、腺瘤组和大肠癌组中的表达水平逐渐下降3,组间表达差异显著(P<0.01);腺瘤组中,高级别上皮内肿瘤其表达率低于低级别上皮内肿瘤(P<0.01);DR-nm23蛋白表达不仅与淋巴结转移呈负相关(P<0.05),而且与肿瘤病理分级及组织学分型相关(P<0.01)。原发癌与淋巴结转移癌中DR-nm23基因及其蛋白表达差异均无显著性(P>0.05)。结论 DR-nm23参与大肠癌细胞的分化,其表达下调与大肠癌发生发展密切相关,可作为判断大肠癌生物学行为及其预后的潜在重要指标。Objective To investigate the expression and significance of DR-nm23 mRAN and protein in colorectal carcinomas.Methods 98 cases of colorectal carcinoma tissues,57 cases of adenoma tissues and 42 cases of normal colorectal tissues were examined by SP immunohistochemical staining and in situ hybridization.Results DR-nm23 mRAN was gradual down-expression in normal group,colorectal carcinoma group,and adenoma group,there existed significant difference in three groups（P0.01）.The positive rate was no significantly lower in high grade intraepithelial tumor than that in low grade intraepithelial tumor in adenoma group（P0.05）.The expression of DR-nm23 mRAN was negatively related to lymph node metastasis（P0.01）.There was closely related to differentiation grade（P0.01）,but there was no closely related to histological types（P0.05）.DR-nm23 protein was gradual down-expression in normal group,adenoma group,and colorectal carcinoma group,there existed significant difference in three groups（P0.01）.The positive rate was significantly lower in high grade intraepithelial tumor than that in low grade intraepithelial tumor in adenoma group（P0.01）.Not only the expression of DR-nm23 protein was negatively related to lymph node metastasis（P0.05）,but also the expression of DR-nm23 protein was closely related to histological types and differentiation grades（P0.01）.There existed no significant difference between in situ tumor and in metastatic tumor of colorectal carcinomas both DR-nm23 mRNA and its protein（P0.05）.Conclusion DR-nm23 participates cell differentiation of colorectal carcinomas.DR-nm23 is closely related to tumorigenesis,progression and metastasis in colorectal carcinomas,which is a potentially important indication for the biological behaviors and prognosis of colorectal carcinomas.
