乳腺癌C-erbB-2表达在新辅助化疗疗效中的预见性

摘　　要：目的比较分析新辅助化疗前后乳腺癌组织中C-erbB-2表达的差异性,探讨其对化疗疗效的预测价值。方法应用Elivision法分别测定53例乳腺癌组织化疗前后C-erbB-2的表达,比较其差异性;同时评估C-erbB-2与化疗疗效的相关性。结果 53例乳腺癌患者新辅助化疗后,病理缓解(包括部分及完全缓解)41例(77.4%),其中病理完全缓解3例(5.7%)。化疗后病理完全缓解的3例无癌组织进行检测,其余50例患者中各指标的变化情况:C-erbB-2新辅助化疗前后表达一致的为43例(86.0%),其差异无统计学意义。C-erbB-2表达阳性的病例86.7%(13/15)、表达阴性的病例73.7%(28/38)有化疗反应,统计学分析无差异性(P=0.472)。结论新辅助化疗不影响乳腺癌组织中C-erbB-2的表达,单因子C-erbB-2的表达与新辅助化疗的疗效无相关性。Objective To study the effect of neoadjuvant chemotherapy on expressions of C-erbB-2 and the value of C-erbB-2 predicting the response rate to chemotherapy in breast cancer.Methods Expressions of C-erbB-2 of 53 patients with breast cancer were measured by Elivision imunohistochemistry before and after neoadjuvant chemotherapy.Pathologic response rates were measured after operative therapy.It was analysed statistically that the correlation between C-erbB-2 and the response rate to neoadjuvant chemotherapy.Results Among all the 53 cases,neoadjuvant chemotherapy achieved clinical response in 77.4% of patients and complete pathologic response in 5.7%.Of 50 patients with complete pathological data,the status of C-erbB-2 were not changed in 43（86.0%） cases.There was no statistical difference significantly before and after neoadjuvant chemotherapy.There was no statistical difference in the response rate to chemotherapy between group with C-erbB-2 ＋ and C-erbB-2（P= 0.472）.Conclusion Neoadjuvant chemotherapy doesn＇ t change the expression of C-erbB-2,and C-erbB-2 positive tumours was not independent predictor of the efficacy of neoadjuvant therapy.
