摘  要:目的探讨强迫症患者的血清脑源性神经营养因子(BDNF)水平的特点。方法收集符合DSM-IV强迫症的诊断标准的患者51例(病例组)及30名正常对照者,其中强迫症(病例组)患者根据临床特征分为非难治性组与难治性组。强迫症病例组中未用药者(未曾用药或停药2周以上者)共12人。采用耶鲁-布朗强迫量表(Y-BOCS)和状态-特质焦虑问卷(STAI)/自尊评定量表(SES)进行评分;采用ELISA法测定血清BDNF的质量浓度。结果①难治性组与非难治性组患者BDNF水平比较无显著性差异(P>0.05);②病例组BDNF水平(13.89±8.88 ng/ml)明显性低于正常对照组(25.08±10.02 ng/ml);③未用药者BDNF水平(8.82±6.14 ng/ml)比正在治疗的病人的BDNF水平(15.45±9.06 ng/ml)低,两者比较差异有统计学意义(P<0.05);④BDNF水平与病程及各量表评分均无明显相关性(r=-0.27~0.20,P均>0.05)。结论强迫症患者的血清BDNF水平明显低于正常对照组,提示BDNF可能参与强迫症的病理作用,血清BDNF水平可能为强迫症的生物学标志之一。Objective To investigate serum brain-derived neurotrophic factor levels in patients with obsessive-compulsive disorder.Methods Collected 51 patients meeting the criteria for obsessive-compulsive disorder(OCD)of DSM-IV and 30 normal controls were enrolled.The patients were divided into two groups depending on clinical characteristics,one was non-refractory obsessive-compulsive disorders and one was refractory obsessive-compulsive disorders.Among the patients there were 12 patients whom had no history of antipsychotic drug use or those with drug washout for more than 2 weeks.Assessment was performed with Yale-Brown Obsessive-Compulsive Severity Scale(Y-BOCS)/State-Trait Anxiety Inventory(STAI)/ Self-esteem Scale(SES),and serum BDNF levels were detected by ELISA.Results ①We found that BDNF levels between refractory obsessive-compulsive disorders and non-refractory obsessive-compulsive disorders were not significantly different(P0.05);②Our findings revealed that the serum BDNF levels in OCD patients(13.89±8.88 ng/ml)were significantly lower than in normal control group(25.08±10.02 ng/ml,P0.01);③The serum BDNF levels of drug-naive OCD patients(8.82±6.14 ng/ml)were lower than those of being treated patients(15.45±9.06 ng/ml,P0.01);④The disease severity,state anxiety inventory(SAI)scores,Self-esteem Scale(SES)scores and disease duration were not related with serum BDNF levels(r=-0.27~0.20,all P0.05).Conclusion In this study,we could conclude that serum BDNF levels in OCD patients were significantly lower than in controls.These data corroborated the hypothesis of a dysfuntion in the neurotrophin expression in the OCD pathogenetic mechanism and provided the rationale for further investigations directed to the identification of novel biomarkers.
