摘 要:目的观察脊髓水平胶质细胞代谢抑制剂氟代柠檬酸对坐骨神经慢性结扎(CCI)大鼠热缩足潜伏期(thermal withdrawal latency,TWL)和机械缩足阈值(mechanical withdrawal threshold,MWT)的影响。方法所有的大鼠术前8 d鞘内置管,用MWT和TWL分别评价大鼠机械痛敏和热痛敏。前给药组:于坐骨神经结扎前1 d开始持续到术后1 d(每天2次)分别鞘内注射生理盐水10μl或氟代柠檬酸50μg,热缩足潜伏期和机械缩足阈值分别于术前2 d,术后1、3、5、7、14 d测定;后给药组:坐骨神经结扎后7 d分别鞘内注射1次生理盐水10μl或氟代柠檬酸50μg,分别于给药后0.5、1、2、4、8 h测定热缩足潜伏期和机械缩足阈值。结果在前给药组中,与前生理盐水组相比,氟代柠檬酸组大鼠在神经结扎后的1~5 d MWT和TWL明显增加(P〈0.05,P〈0.01);后给药组中,氟代柠檬酸组大鼠在给药后的各个时间点与后生理盐水组和给药前自身相比,在给药后0.5、1、2、4 h MWT和TWL明显增加(P〈0.05,P〈0.01)。结论术前鞘内注射氟代柠檬酸明显延迟CCI大鼠热痛敏和机械痛敏形成,并且术后7 d鞘内注射氟代柠檬酸明显抑制CCI大鼠已建立热痛敏和机械痛敏,提示胶质细胞的活化参与慢性坐骨神经结扎引发神经病理痛形成和维持。Objective To observe the effect of fluorocitrate,a glial inhibitor,administered intrathecally on thermal withdrawal latency(TWL) and mechanical withdrawal threshold(MWT) of rats with chronic constriction injury(CCI) of the sciatic nerve. Methods SD rats in all groups were intrathecally cathetered at 8 d before operation.Mechanical allodynia and heat hyperalgesia were evaluated by MWT and TWL,respectively.For the rats receiving pre-treatment,glial inhibitor fluorocitrate(50 ug) or normal saline(10 ul),beginning on 1 d before operation,were intrathecally administered twice daily for consecutive 3 days(pre-treatment),the values of TWL and MWT were measured prior to sciatic nerve ligation and at 1,3,5,7,and 14 d after surgery.For the rats receiving post-treatment,fluorocitrate(50 ug) or normal saline(10 ul) were intrathecally administered once on the seventh day after ligation,the values of TWL and MWT were examined at 0.5,1,2,4,and 8 h after intrathecal injection. Results In pre-treatment group,there was significantly increasing in MWT and TWL in group receiving fluorocitrate from day 1 to day 5 post ligation(P〈0.05,P〈0.01).As compared with those at different time points in post-treatment group receiving normal saline,the MWT and TWL in group receiving fluorocitrate significantly increased at 0.5,1,2,and 4 h post drug(P〈0.05,P〈0.01). Conclusions Intrathecal treatment with fluorocitrate significantly delays the occurrence of thermal hyperalgesia and allodynia or suppresses the established thermal hyperalgesia and allodynia in CCI rats.The results suggest that glial activation is involved in the forming and maintenance of neuropathic pain induced by CCI.
