手助腹腔镜结直肠切除术的临床观察研究

摘　　要：目的比较手助腹腔镜与开腹结直肠切除术的临床效果,评价手助腹腔镜手术在结直肠肿瘤手术治疗中的应用价值。方法将收治的65例患者分为手助腹腔镜手术(30例)和开腹手术(35例),分析两组的临床效果。结果腔镜组有3例中转开腹,两组在术中出血量、肠道功能恢复时间及术后住院时间的差异有统计学意义(P<0.05)。在手术时间、术后并发症发生率、术后复发率的差异无统计学意义(P>0.05)。结论手助腹腔镜直肠癌根治术具有安全性高、手术时间短、创伤小、出血少、术后恢复快等优点。Objective To compare the clinical effects of hand-assisted laparoscopic surgery and open operation in colorectal resection,and to assess the application value of hand-assisted laparoscopic surgery in the treatment of colorectal cancer.Methods Sixty-five patients with rectal carcinoma were divided into hand-assisted laparoscopic surgery（HALS group,30 cases） and open（35 cases） groups,and the clinical effects of two groups were analyzed.Results Three patients in the HALS group were converted to open surgery.There existed statistically significance in peri-operative bleeding,return of bowel function and hospital post-operative between the two groups（P0.05）,but not in the operation time,the rate of post-operative complication and postoperative recurrence rate（P0.05）.Conclusion Hand-assisted laparoscopic radical resection of rectal carcinoma has the advantage of high safety,short operation time,slight invasion,less bleeding and rapid post-operative recovery,etc.
