中心性浆液性脉络膜视网膜病变FFA和FAF图像的对比分析

摘　　要：目的研究中心性浆液性脉络膜视网膜病变(central serous chorioretinopathy,CSC)患者眼底自发荧光(fundus autofluo-rescence,FAF)图像及荧光素眼底血管造影(fundus fluorescein angiography,FFA)的特征。方法 应用共焦激光扫描检眼镜对43例(48眼)CSC患者进行检查,分析其FAF与FFA图像特点。结果 FFA检查:急性期CSC41眼FFA检查均有视网膜色素上皮(retinal pigment epithelium,RPE)荧光素渗漏,2眼伴RPE脱离;7眼慢性迁延性CSC的FFA表现为黄斑区斑驳样透见荧光,造影晚期可见荧光素缓慢渗漏。FAF特征:急性期CSC盘状脱离区呈单纯性的低自发荧光区、脱离区呈低自发荧光而其周围为斑点状强自发荧光环、脱离区呈强自发荧光池、脱离区呈点状强自发荧光等改变;RPE脱离表现为低自发荧光或轻度增强的自发荧光。渗漏点的改变:渗漏点处呈单纯低自发荧光、低自发荧光而其周围为局部强自发荧光、自发荧光无明显改变等。慢性迁延性CSC的FAF图像表现为斑点状强弱自发荧光相互交织。结论 CSC患者FAF与FFA图像相比较为复杂,FAF可用于检测RPE细胞的功能状态及CSC的辅助检查。Objective To investigate the image characteristics of fundus autofluorescence（FAF） and fundus fluorescein angiography（FFA） in patients with central serous chorioretinopathy（CSC）.Methods FAF and FFA images were recorded by confocal laser scanning ophthalmoscope in 48 eyes of 43 subjects with CSC,and analyzed the image characteristics.Results FFA images showed that 41 acute CSC eyes revealed retinal pigment epithelium（RPE） leakage,2 eyes combined with pigment epithelial detachment（PED）;Seven chronic CSC eyes showed mottled like RPE damage and weak fluorescein leakage at later stage.FAF images showed that the detached area of acute CSC changed,including single hypoautofluorescence area,hypoautofluorescence around the hyperautofluorescence,hyperautofluorescence pool,dotted hyperautofluorescence;PED showed hypoautofluorescence or slight hyperautofluorescence.Leakage of FFA showed three forms on FAF,including without significant autofluorescence changes,simple hypoautofluorescence points and hypoautofluorescence points surrounded by local hyperautofluorescence.Chronic CSC eyes in FAF image showed speckled hyperautofluorescence combined with hypoautofluorescence in macula.Conclusions The FAF characteristics of CSC is more complicated compared with FFA.FAF can be used to detect functional status of RPE cells and auxiliary examination for CSC.
